OSAHS患者腭咽组织病理细胞学改变与瘦素表达相关性研究

摘　　要：目的初步探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者腭咽组织病理细胞学改变与瘦素表达的相关性。方法 OSAHS患者30例及非OSAHS行咽喉部手术的肥胖病人组30例,手术中切取扁桃体及双侧舌腭弓游离缘少量组织,约0.1 g。进行HE染色,观察炎症细胞浸润情况,SP法免疫组化染色,观察瘦素在组织细胞中的阳性表达部位及强度。结果腭咽组织多量淋巴、浆细胞、脂肪细胞浸润;小血管增生伴充血;肌纤维萎缩、增生、水肿伴淋巴细胞、浆细胞浸润。瘦素在OSAHS组和对照组均有阳性表达,无显著性差异,而不同严重程度OSAHS患者瘦素表达则有显著性差异;OSAHS组与对照组腭咽组织炎症反应无显著性差异,不同严重程度OSAHS患者腭咽组织的炎症反应亦无显著相关性;OSAHS组腭咽组织瘦素表达与炎症反应之间无明显相关性。结论阻塞性睡眠呼吸暂停低通气综合征患者腭咽部的炎症反应可能与打鼾时的振动损伤、克服上气道阻力的用力呼吸对黏膜的创伤、HP感染等有关,与瘦素表达之间无明显相关性,具体机制尚需要进一步研究。Objective To study the correlation between pathologic changes and expression of leptin in the pharyngeal tissue of patients with obstructive sleep apnea-hyponea syndrome. Methods 30 patients with OSAHS were laken as the OSAHS group and 30 obese people without OSAHS were selected as the control group. Tonsil and bilateral lingual arch free margin tissue （approximately 0. 1 g） were cut by operatkm. HE staining was used to ob- serve the inflammatory cells infiltration. The expression of leptin in the tissue was investigated by SP staining tech- nique. Results There were many lympholeukocyte, plasmaeyte and fat cells. Small blood vessels were complicated with hyperplasia and congestion. The expression of leptin was positive in the two groups, and the expression of leptin in the OSAHS group was closely correlated with severity. The inflammation in the pharyngeal tissue had no significant difference between the two groups. There was no significant correlation between the expression of leptin and the in- flammation. Conclusion Snoring vibration damage, mucosal trauma by forced respiration to Overcome the upper air- way resistance and infection of HP may be correlated with the inflammatory reaction in the pharyngeal tissue of pa- tients with OSAHS, and there is no significant correlation between pathologic changes and the expression of leptin in the pharyngeal tissue.
