摘　　要：目的评价2型糖尿病患者应用出院计划服务的效果。方法将100例2型糖尿病患者随机分为对照组和观察组各50例。对照组接受常规教育和随访,观察组在此基础上实施出院计划服务。结果出院3个月后观察组自我管理行为得分显著高于对照组,空腹血糖、餐后2h血糖及糖化血红蛋白显著低于对照组(均P<0.01)。结论出院计划服务可提高2型糖尿病患者的自我管理行为,有效控制血糖。Objective To evaluate the effects of discharge planning service on patients with type 2 diabetes. Methods A total of 100 patients with type 2 diabetes were randomly assigned into a control group and an observation group of 50. The control group received conventional health education and follow-up service, while patients in the observation group were given discharge planning service for 3 months. The scores of self-management behaviors, blood glucose and glycosylated hemoglobin（HbAlc） were measured before and after 3 months. Results Self-management ability was significantly higher and the values of fasting blood glucose, postprandial 2 h blood glucose, and HbAlc were statistically lower in the observation group than those in the control group in 3 months after discharge （P〈0.01 for all）. Conclusion Discharge planning service can enhance self-management behaviors of type 2 diabetic patients and maintain better glycemic control.
