瑞芬太尼静脉应用于老年腹部手术患者的临床分析

摘　　要：目的:观察静脉应用瑞芬太尼在老年腹部手术患者麻醉中的临床效果和安全性。方法:选择我院腹部手术的老年患者78例,随机分为观察组和对照组,分别应用瑞芬太尼麻醉和芬太尼麻醉,比较两组术中心率、血压的变化以及术后拔管时间及认知能力恢复情况。结果:两组麻醉诱导后血压和心率均较诱导前降低,差异具有统计学意义(P<0.05);观察组插管时和切皮时心率和血压较诱导后无明显变化,差异无统计学意义(P>0.05);对照组插管时和切皮时心率和血压较诱导后升高,差异具有统计学意义(P<0.05);观察组拔管时间和认知能力恢复时间均短于对照组,差异具有统计学意义(P<0.05)。结论:瑞芬太尼静脉麻醉用于老年患者血流动力学更平稳,术后苏醒快,麻醉效果及安全性较好,适合临床选择应用。Objective：To observe the clinical effect and safety of intravenous Remifentanil for anesthesia in senile patient undergoing abdominal operation.Methods：A total of 78senile patients scheduled for abdominal operation were chosen and divided into observation group and control group randomly, which anesthetized by remifentanil and fentanyl,respectively.The heart rates,blood pressures,extubate time and cognition recovery time of patients in two groups were observed and compared.Results：The heart rates and blood pressure of patients in two groups were both reduced after anesthesia induction compared with that before anesthesia induction（P0.05）.The heart rates and blood pressure were not apparently changed at start of intubation and cutting in observation group（P0.05）,while they were significantly elevated in the control group（P0.05）.The time of withdrawal and cognition recovery were much shorter in observation group than those in control group（P0.05）.Conclusion：Intravenous Remifentanil anesthesia used in senile patient can keep hemodynamic more smooth,and cognition restore time short after anesthesia.It is efficient and safe thus is suitable for clinical application.
