肺表面活性物质联合鼻塞持续正压气道通气治疗新生儿呼吸窘迫综合征疗效观察

摘　　要：目的评价肺表面活性物质(PS)联合鼻塞持续正压气道通气(NCPAP)治疗新生儿呼吸窘迫综合征(NRDS)的疗效。方法将68例NRDS患儿分为2组,治疗组给予PS联合NCPAP治疗,对照组给予单纯NCPAP治疗,观察2组NCPAP参数变化、血气分析、肺氧合功能变化、并发症及住院情况。结果治疗组总治愈率87%,明显高于对照组的62%;治疗后2组1 h、6 h、12 h、24 h、48 h血气分析及肺氧合功能等均得到改善,2组间比较无显著性差异。供氧浓度、供氧时间、并发症、住院时间、住院费用2组间比较有显著性差异(P均<0.05)。结论 NCPAP联合PS治疗NRDS效果较好,用氧时间、住院时间短,可减少住院费用,降低并发症发生率,提高治愈率。Objective It is to evaluate the curative effect of pulmonary surfactant （PS） combined with nasal continuous positive airway pressure ventilation （NCPAP） on neonatal respiratory distress syndrome （NRDS） effect. Methods 68 patients with NRDS were divided into two groups, the treatment group was treated with PS combined with NCPAP, the control group was simply treated with NCPAP, NCPAP parameters, blood gas analysis, oxygenation function, complications and hospitaliza-tion situation were observed in both groups. Results The total cure rate was 87% in treatment group, which was significantly higher than that of 62% in control group, blood gas analysis, oxygenation function were both improved in both groups in 1 h, 6 h, 12 h, 24 h, 48 h after treatment, compared with that before treatment, the difference was statistically significant （ P 〈 0.01 ） in each group, but the difference between the two groups was not statistically significant. There were significant differ-ent in oxygen concentration, oxygen supply time, complications, hospital stay, hospital charges between the two groups（ P 〈 0. 05 ）. Conclusion NCPAP combined with PS has a good curative effect on NRDS, which can shorten the oxygen using time, hospitalization time and reduce hospitalization costs and complication occurrence rate, and increase curing rate.
