摘  要:目的:探讨妊娠期糖尿病孕妇外周血及胎盘组织NF-κB表达与胰岛素抵抗的相关性。方法:选择孕中晚期糖尿病孕妇120例为观察组,正常孕妇60例为对照组,对两组空腹血糖(FPG)、胰岛素(FINs)、胰岛素抵抗指数(HOMAIR)、核因子-κB(NF-κB)活性以及E-选择素(sE-let)和可溶性细胞间粘附分子-1(sICAM-1)水平进行观察和比较。结果:与对照组相比,观察组FPG、FINs、HOMA-IR均明显升高,sE-let、sICAM-1及NF-κB水平均明显升高,差异均有统计学意义(P<0.05),并且NF-κB活性与sE-let、sICAM-1、FINs及HOMA-IR水平呈正相关性。结论:妊娠期糖尿病患者外周血NF-κB活性明显升高,可能通过上调sE-let和sICAM-1表达参与胰岛素抵抗的发生、发展。Objective: To study the correlation between the NF-κB expression in maternal blood and placental tissue of gestational diabetes mellitus pregnants and the insulin resistance. Methods: 180 late pregnants were randomized into the control (normal pregnants, n = 60) and the observation groups (gestational diabetes mellitus, n = 120) . The fasting plasma glucose (FPG), insulin (FINs), insulin resistance index ( HOMA - IR), nuclear factor-κB activityin mononuclear cells ( NF-κB), sE - selectin ( sE - let) and souble intercellular adhesion molecule - 1 ( sICAM - 1 ) were observed and compared in the two groups. Results: Compared with the control group, the FPG, FINs, HOMA - IR were significantly increased, the levels of sE - let, sICAM - 1 and NF-κB were significantly increased in the observation group ( P 〈 0.05 ), and the NF-κB activity was postively correlated with sE - let, sICAM - 1, HOMA - IR and FINs. Conclusion: The NF-κB activity in the gestational diabetes mellitus pregnants is significantly increased, may up - regulate the expression of sE - let and sI- CAM - 1, and involve in the occurrence and development of insulin resistance.
