摘　　要：目的:为减少静脉用药调配中心工作差错提供参考。方法:对静脉用药调配中心工作中出现的典型差错按其在工作流程中出现的先后顺序进行回顾性分析总结,制订改进措施。结果与结论:差错分布在审方、摆药、复核、配制、药品管理几个环节;通过对典型案例进行分析,找出问题的关键,确立了差错的处理重点是优化工作流程,加强复核确认和人员培训,建立合理的差错制度,逐步完善静脉药物调配各环节的岗位职责。OBJECTIVE： To provide reference for Pharmacy intravenous admixture service （PIVAS） staff to reduce and avoid mistakes. METHODS： The typical Cases of the drug dispensing and preparation errors in PIVAS department were analyzed and sum- marized retrospectively, and the corresponding preventive measures were proposed. RESULTS ＆ CONCLUSIONS： The errors are mainly concentrated in prescription-checking, drug dispensing, drug-checking, intravenous admixture and drug administration proce- dures. To avoid the errors in PIVAS, it should focuse on workflow optimization, re-checking and personnel training, the establish- ment of reasonable error handling system, responsibilities improvement of intravenous drugs dispensing in each link.
