血清视黄醇结合蛋白(RBPs)和尿N-乙酰-β-D氨基葡萄糖苷酶(NAG)在糖尿病肾病诊断中的价值

摘　　要：目的探讨血清视黄醇结合蛋白(retinol binding proteins,RBPs)和尿N-乙酰-β-D氨基葡萄糖苷酶(N-acetyl-β-D-glucosaminidase,NAG)在糖尿病肾病(diabetic nephropathy,DN)诊断中的价值。方法选取2010年5月~2012年5月我院进行治疗的105例2型糖尿病患者,以尿微量白蛋白(mALB)/尿肌酐(UCr)比值将患者分为研究组和对照组。结果研究组血清RBPs、尿NAG水平与对照组比较显著增高,差异有统计学意义(P<0.05)。研究组血清RBPs阳性为24.53%,对照组为3.85%,两组比较,差异有统计学意义(P<0.05)。研究组尿NAG阳性率为30.19%,对照组为7.69%,两组比较,差异有统计学意义(P<0.05)。105例2型糖尿病患者尿mALB与血清RBPs、尿NAG水平呈正相关关系(r=0.539,r=0.447,P<0.05)。结论在糖尿病早期阶段,肾小管功能会受到损害,血清RBPs和尿NAG作为早期DN诊断的敏感指标,具有重要价值。Objective To explore the diagnosis value of serum retinol binding proteins （RBPs） and urine N- acetyl beta-D glucosaminidase （NAG） in diabetic nephropathy. Methods 105 patients with diabetes were enrolled from May 2010 to May 2012 in our hospital. According to the urine micro albumin （mALB） / creatinine （UCr） ratio, the patients were divided into study group and control group. Results In study group, serum RBPs and urine NAG levels were significantly increased, compared to the control group, the difference was statistically significant （P〈0.05）. The positive rate of serum RBPs of study group was 24. 53%, higher than 3.85% in the control group, the difference was statistically significant （P〈0.05）. The positive rate of urine NAG in study group was 30.19%, higher than 7.69% in the control group, the difference was statistically significant （P〈0.05）. In 105 cases of type 2 diabetic patients serum RBPs levels were positively correlated with urine NAG and mALB levels（r= 0. 539, r= 0. 447, P^0.05）. Conclusion In the early stage of diabetes, renal function damage, serum RBPs, urine NAG can be used as sensitive indicates for diagnosis of early diabetic nephropathy in type 2 diabetes mellitus.
