摘  要:目的探讨多囊卵巢综合征(PCOS)患者血管内皮功能损伤的情况,以及其与代谢性指标相关性的研究。方法30例PCOS患者和20例的正常妇女(对照组)为研究对象;于月经期第3天抽取空腹静脉血去血清,检测血清选择素E(E—selectin)、内皮素-1(ET-1)和血管内皮vWF因子水平及总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL—C)和高密度脂蛋白胆固醇(HDL-C)的水平变化;测定空腹胰岛素及血糖(FBG)水平。结果PCOS组血浆E—selectin和vWF、TG和LDL-C、血浆空腹血糖和胰岛素水平明显高于正常对照组(P均〈0.05)。PCOS患者血浆E.selectin与血糖、胰岛紊及血三酰甘油呈正相关,r值分别为0.44、0.56和0.61,P均〈0.05;尚不能认为与血胆固醇相关。PCOS患者血浆vwF与血糖、胰岛素及血三酰甘油呈正相关,r值分别为0.48,0.52和0.63,P均〈0.05:尚不能认为与血胆固醇相关。PCOS组患者血ET-1仅与总胆固醇呈正相关性,r值为0.49,P〈0.05。结论PCOS患者中E.selectin和vwF的升高提示了其血管内皮功能受损,血糖、高胰岛素和血脂升高是PCOS中影响血管内皮损伤的重要因子。Objective To investigate the endothelial dysfunction in patients with polycystic ovary syndrome(PCOS) and its correlation with metabolic status. Methods Thirty patients with PCOS and 20 normal women( NGT group) were enrolled in the study. Fasting blood samples were drawn from the vein on the third day of the menstrual cycle. Plasma levels of ET-1, vWF and soluble E-selectin were detected by ELISA. Lipid metabolism (total cholesterol, HDL-cholesterol, LDL-cholesterol, triglycerides) and glucose were also detected. Results The study showed higher levels of vWF, E-selectin, insulin and glucose and TG, LDL in PCOS patients versus healthy women ( all P 〈 0.05 ). A positive correlation was demonstrated between E-selectin and glucose, triglycerides and insulin ( all P 〈 0. 05 ). vWF levels showed a positive correlation with glucose and triglycerides ( all P 〈 0. 05 ). A positive correlation was also found between ET-1 and total cholesterol levels in PCOS women (P 〈 0. 05 ). Conclusions Elevated levels of vWF :Ag and E-selectin in PCOS women suggest endothelial dysfunction in this group of patients. Glucose and hyperinsulinism, hyperlipemia are significant factors affecting endothelial function in PCOS.
