摘  要:目的:评价缬沙坦治疗轻、中度原发性高血压的临床疗效及安全性。方法:缬沙坦组(59例,80 mg/d至160 mg/d)和氯沙坦组(对照组,58例,50 mg/d至100 mg/d)均连续服药半年。结果:两组总有效率、降压幅度、收缩压和舒张压的各/峰比值分别为75%和 74%、 3.5 kPat2.0 kPa和 3.4 kPa/2.1kPa、0.82和0.81、0.78和0.79,组间比较均无显著性差异(P>0.05);两组治疗后血浆肾素活性、血管紧张素Ⅱ均升高(P<0.05~0.01);醛固酮、内皮素均降低(均为P<0.05),但组间比较无显著性差异(P>0.05)。血尿酸也明显下降(均为P<0.05),基础值越高,降幅越大,且氯沙坦组更显著(P<0.05)。随访半年,两组心、脑血管事件发生率相似(P>0.05),两组不良反应轻微。结论:缬沙坦的疗效与氯沙坦相近,值得临床推广应用。Objective: To evaluate the clinical effects and safety of treating mild and moderate essential hypertension with valsartan. Methods: One hundred and seventeen cases were assigned into two groups, one were treated with valsartan 80 mg^160 mg, another with losartan 50 mg^l00 mg once daily for half a year. Results: The total effective rate, amplitude of blood pressure reduced, T/P ratio of systolic and diastolic blood pressure in valsartan and losartan group were 75% and 74%, 3.5 kPa/2.0 kpa and 3.4 kPa/2.1 kPa, 0.82 and 0.81, 0.78 and 0.79 respectively. No significant differences existed between two groups (P>0.05). Plasma renin activity (PRA) and angiotensin Ⅱ elevated after the therapy (P <0.05~0.01), while aldosterone and endothelin (ET) decreased (P<0.05) in both groups, there were no significant differences between two groups (P >0.05). Uric acid level of blood also reduced after the treatment, and the higher the level before the therapy, the larger the amplitude reduced. The amplitude reduced was larger in losartan group (P<0.05). The cardiocerebral events of two groups were similar during therapy. Adverse reactions in valsartan group were mild. Conclusion: Valsartan is a new angiotensin Ⅱ type 1 receptor (AT1) antagonist with similar effect of losartan and worths recommending in clinical practice..
