乳腺黏液囊肿样病变超声表现及病理特征

摘　　要：目的探讨乳腺黏液囊肿样病变(MLL)的超声表现及病理特征。方法回顾性分析经病理确诊的10例患者15个病灶超声表现及病理特征。结果 12个病灶位于左侧乳房,3个位于右侧乳房。超声表现8个病灶共同特点为囊状无回声,其中心或附壁可见等回声影,多数等回声内伴微钙化,彩色多普勒显示附壁等回声内部可见点状血流信号;4个病灶呈多发分隔囊状回声;3个病灶无回声内部可见密集点状回声。术后病理显示高度扩张的囊腔,其内部分或全部充满黏液,同时部分黏液内可见多发微钙化。结论 MLL主要超声表现为囊状无回声,内部或附壁可见等回声,其内可伴有多发微钙化,确诊需结合病理结果。Objective To observe ultrasonographic presentations and histopathological characteristics of mucocele-like lesions of the breast.Methods Ultrasonographic presentations and histopathological characteristics of 15 lesions in 10 patients with MLL confirmed by pathology were retrospectively reviewed.Results Twelve lesions located in the left and 3 in right breast.Anechoic liquid area were found in 8 lesions and iso-echoic nodule were found in its core or wall.There were microcalcifications in most of iso-echoic nodules.CDFI showed point flow signals in iso-echoic nodule.Fibrous septums were found in 4 lesions; Thick point echo was found in 3 anechoic liquid areas.The histopathologic examination showed dilated cysts were noted.Whole or part of cyst were filled with mucin,which could be found microcalcifications in it.Conclusion MLL has some ultrasonic characteristics,including anechoic liquid area,iso-echoic nodule in wall or core,and microcalcifications,but the diagnosis still depends on histopathology.
