骶髂关节MR特征对强直性脊柱炎生物制剂疗效的观察

摘　　要：目的探讨强直性脊柱炎(AS)在治疗过程中,使用MR检查研究骶髂关节(SIJ)骨髓水肿的变化与临床活动性指标变化的相关性。方法选择8例患者应用生物制剂(益赛普或恩利)。分别在用药前、后3、6、9、12个月进行检查。MR检查采用SPARCC评分,同时测量T2WI信号强度,应用BASDAI评分评估AS病情活动指数,实验室检查ESR及CPR,评估SPARCC评分变化值、T2信号强度变化值与BASDAI评分、ESR及CPR变化值的相关性,同时评估ESR及CPR变化值的相关性。结果结果显示SPARCC评分变化值与BASDAI评分变化值呈正相关(r=0.888,P<0.05);T2信号变化值与BASDAI评分变化值呈正相关(r=0.670,P<0.05)。结论 SPARCC评分变化值、T2信号强度变化值可以定量的检测AS患者的治疗效果,因而为患者提供一个可视性的观察指标。Objective To investigate the correlation between the changes in bone marrow edema and and the alterations in clinical active indexes during the course for the treatment ankylosing spondylitis （AS） via MR imaging examination. Methods 8 patients, who were subjected to biological agents, i. e. , Etanercept or Andes, were selected in this work. The inspection was performed 3, 6, 9, 12 months pre-and post-treatment, respectively. SPARCC score was adopted for MR examination and T2WI signal intensity was measured. By use of the bath ankylosing spondylitis disease activity index （BASDAD score, the correlation of the changes of the SPARCC score, T2 signal intensity and BASDAI score were as sessed and, meanwhile, the correlation of change between ESR and CPR was also determined. Results A positive correla tion between the changes of SPARCC score and BASDAI score was found （ r =0. 888, P %0.05）. Whereas, a negative correlation between the changes of T2 signal and BASDAI score was existed （ r =0. 670, P %0. 05）. Conclusion The changes in SPARCC score and in T2 signal intensity can quantitatively detect the efficacy of the treatment for AS patient, thereby providing a visibility indicator for the observation of these patients.
