血清MMP-1、MMP-9、TIMP-1、IL-6水平与冠状动脉粥样硬化相关性研究

摘　　要：【目的】评估血清基质金属蛋白酶-l（MMP-1）、基质金属蛋白酶-9（MMP-9）、组织金属蛋白抑制因子-1（TIMP-1）和白介素-6（IL-6）水平和冠状动脉粥样硬化性心脏病（CAD）严重性之间的关系。【方法】连续性冠状动脉造影的患者100例，据病情分为5组：稳定型心绞痛（sAP）20例，不稳定型心绞痛（USAP）19例，非ST段抬高心肌梗死（NSTEMI）10例，急性ST段抬高心肌梗死（STEMI）2l例和对照组（冠状动脉造影正常）30例。ELISA法检测患者MMP-1、MMP-9、TIMP-1、IL-6的血清浓度，计算冠状动脉造影Gensini评分。【结果】USAP组、SAP组与对照组，STEMI组与NSTEMI组}E较MMP-1水平较高；STEMI组MMP-9水平明显高于USAP组和对照组；5组间TIMP-1水平没有差异（P〉0．05）。糖尿病患者MMP-1的水平升高，吸烟者体内MMP-9水平更高。MMP-1、MMP-9、IL-6水平与左前降支（I。AD）狭窄和Gensini评分存在相关性。【结论】血清MMP-1，MMP-9和IL-6水平与CAD疾病严重性相关。[Objective] To evaluate the correlation between serum levels of MMP-1, MMP-9, TIMP-1, IL 6 and the severity of atherosclerotic coronary artery disease（CAD）. [Methods] According to the degree of the disease, 100 consecutive patients undergoing coronary angiography were divided into 5 groups including stable angina pectoris group（SAP group, n = 20）, unstable angina pectoris group（USAP group, n = 19）, non-ST elevation myocardial infarction group（NSTEMI group, n = 10）, acute ST elevation myocardial infarction group（STEMI group, n=21） and control group（normal coronary angiography group, n =30）. Serum levels of MMP-1, MMP-9, TIMP- 1 and IL-6 were detected with ELISA method. Meanwhile coronary angiographic Gensini score was calculated. [Results] Compared with USAP group, SAP group and control group, MMP-1 levels in STEMI group and NSTEMI group were higher. MMP-9 levels in STEM/group were obviously higher than those in USAP group and control group. There was no significant difference in TIMP-1 among 5 groups. MMP-1 levels in diabetic patients increased, while MMP-9 levels in smokers were even higher. The levels of MMP-1, MMP-9 and IL-6 were correlated with left anterior descending artery（LAD） stenosis and angiographic Gensini score. [Conclusion] Serum levels of MMP-1, MMP-9 and IL-6 are correlated with the severity of CAD.
