自体肋软骨切取手术设计与操作要点

摘　　要：目的探讨自体肋软骨切取手术设计与操作要点。方法 2012年6月至2012年12月于166例耳再造及鼻部整形等手术中进行自体肋软骨切取操作,合理设计手术切口,细致剥离操作,注意保护骺软骨及软骨膜,并于切取后缝合软骨膜。皮肤切口采用整形缝合技术处理。结果手术切取自体肋软骨安全而快速。术后患者均恢复良好,无伤口血肿、感染、愈合不良、气胸等并发症发生。手术切口愈合情况良好。结论采用合理设计原则及细致操作步骤,手术过程耗时短且安全而顺利,能有效防止并发症的发生并达到理想的远期效果。Object To investigate the surgical technique and strategy of autologous costal cartilage harvest. Method Dur- ing 2012.06-2012.12, 166 patients with autologous costal cartilage harvest for rhinoplasty or ear reconstruction were evaluat- ed. Appropriate design of the incision and delicate surgical manipulation were performed, while osteoepiphysis and perichon- drium of costal cartilage should be protected as well. After cartilage harvesting, suture of perichondrium was recommended. The incision of skin was handled with plastic surgical suture technique. Result Both safety and efficiency of operational proce- dure were achieved, without presentation of hematoma, infection, pneumothorax and any other complications. Conclusion Ap- propriate design of the incision and delicate surgical manipulation were the key points of the safety and efficiency of surgical procedure, and also for prevention of complications. Ideal results accomplished post-operationally.
