摘 要:目的通过频域光学相干断层扫描(optical coherence tomography,OCT)观察糖尿病患者中角膜厚度的变化。方法通过眼前段频域OCT对40名(80眼)健康对照者、55例(110眼)2型糖尿病不合并糖尿病视网膜病变(diabetic retinopathy,DR)患者、55例(110眼)2型糖尿病合并单纯性DR患者、33例(66眼)2型糖尿病合并增殖性糖尿病视网膜病变(proliferative diabetic retinopathy,PDR)患者角膜进行扫描,测量角膜全层、上皮层和基质层的厚度。结果对照组角膜全层、上皮层和基质层的厚度分别为(516.9±26.5)μm、(44.2±8.6)μm和(473.8±27.5)μm,糖尿病组患者中各指标分别为(542.8±31.9)μm、(44.7±9.4)μm和(502.9±33.7)μm,糖尿病组患者的角膜全层厚度和基质层厚度与对照组差异均有统计学意义(均为P=0.000)。2型糖尿病不合并DR患者中,角膜全层、上皮层和基质层的厚度分别为(533.2±17.4)μm、(44.1±5.4)μm和(492.5±20.8)μm。2型糖尿病合并单纯性DR的患者中各指标分别为(524.7±22.6)μm、(43.0±8.6)μm和(482.7±28.4)μm。2型糖尿病合并PDR的患者中,各指标分别为(572.1±31.3)μm、(44.7±4.7)μm和(528.9±37.3)μm。糖尿病3个亚组间进行比较,角膜全层和基质层厚度差异均有统计学意义(P=0.001、P=0.000),而角膜上皮层厚度的差异无统计学意义(P=0.250)。结论通过频域OCT可测量角膜全层、上皮层和基质层的厚度。与健康对照者相比,2型糖尿病患者角膜全层和基质层明显增厚,而上皮层厚度无显著改变。在合并PDR的糖尿病患者中,角膜全层和基质层的厚度显著增厚。ObJeetive To observe the changes of corneal thickness by spectral domain optical coherence tomography (OCT) in patients with diabetes mellitus. Meth otis A total of 40 healthy ones (80 eyes) ,55 type 2 diabetes mellitus patients without diabetic retinopathy (DR, 1 l0 eyes) ,55 type 2 diabetes mellitus patients with single DR (ll0 eyes),33 type 2 diabetes mellitus patients with proliferative diabetic retinopathy (PDR,66 eyes) were enrolled. The central corneal thickness including the full layer thickness,epithelial layer and stromal layer were measured by spectral-domain OCT. Results In the healthy control group,the average thickness of full corneal layers, epi thelial layer and stromal layer were (516.9 ± 26.5 ) txm, (44.2 ± 8.6) pxm and (473.8 ± 27.5 ) ixm, respectively, which in the group of diabetes mellitus were (542.8 ± 31.9 ) ixm, (44.7 ± 9.4) txm and (502.9 ± 33.7 ) ixm, respectively. Compared with the control group, there were statistical differences in average thickness of full layers and stromal layer of ' diabetes mellitus group ( all P = 0.000). In the group of diabetes meUitus without DR, ::the mean thickness of full corneal layers, epithelial layer and stromal layer were ( 533.2 ± 17.4 ) um, ( 44. 1 ± 5.4 ) ixm and ( 492.5 ± 20.8 ) um, respectively, the group of diabetes mellitus with single DR were (524.7 ±22.6) ixm, (43.0 +8.6) um and (482.7 ± 28.4) um, respectively, the group of diabetes mellitus with PDR were ( 572.1± 31.3 ) um, (44.7± 4.7 ) Ixm and ( 528.9 ± 37.3 ) um, respectively.
