摘  要:目的探讨甲状腺良恶性结节超声造影的时间-强度曲线(time-intensity curve,TIC)的形态和声学定量参数变化特征。方法对189例患者的213个甲状腺结节(良性结节145个,恶性68个,均经术后组织病理证实)进行超声造影血流灌注增强,以声学定量软件绘制甲状腺良恶性结节的TIC,分析TIC形态和定量参数变化特征。结果超声造影TIC形态表现:甲状腺癌表现为缓慢进入,上升支较平坦,峰值较周边甲状腺组织低;结节性甲状腺肿表现为迅速进入,上升支较陡峭,峰值与周边甲状腺组织一致;甲状腺腺瘤表现为迅速进入,上升支较陡峭,峰值较周边甲状腺组织高。TIC定量参数比较:甲状腺癌、结节性甲状腺肿及甲状腺腺瘤始增时间、达峰时间比较差异均无统计学意义(P>0.05),峰值强度比较差异均有统计学意义(P<0.05)。结论超声造影的TIC形态和定量参数变化特征对甲状腺良恶性结节的诊断及鉴别诊断有重要价值。Objective To explore the types and the acoustic changes of time-intensity curve (TIC) of contrast-enhanced ultrasound in diagnosing benign and malignant thyroid nodules. Methods A total of 213 thyroid nodules (pathologically confirmed 145 benign and 68 malignant) in 189 patients were performed contrast-enhanced ultrasound scanning. Acoustic quantified software was used to draw TIC. The types and the acoustic changes of TIC were analyzed. Results Contrast- enhanced ultrasound of TIC showed slowly entering of contrast into flat ascending branch and lower peak value compared with surrounding thyroid tissue in thyroid carcinoma, rapidly entering of contrast into steep ascending branch and similar peak value with the surrounding tissue in nodular goiter, and rapidly entering of contrast into steep ascending branch and higher peak value compared with surrounding tissue in thyroid adenoma. Quantitative comparison of TIC showed no significant differences in rise time and time to peak (P〉0.05), and peak intensity showed a significant difference among thyroid carcinoma, nodular goiter and thyroid adenoma (P〈0.05). Conclusions The TIC types and acoustic changes of contrast-enhanced ultrasound have important value to the diagnosis and differentiate diagnosis of benign and malignant thyroid nodules.
