鼻腔解剖变异与慢性鼻窦炎关系的研究

摘　　要：目的 :评价鼻腔HRCT检查及临床应用价值。方法 :应用PhilipsSR 70 0 0螺旋CT扫描仪对 10 6例鼻疾病患者行冠状位及横断位HRCT扫描 ,层厚 3mm ,层间距 5mm。其中 12例层厚 3mm ,层间距 3mm。结果 :本组病例慢性鼻窦炎患者占 92 % ,扫描显示鼻中隔偏曲占 81 6% ,中鼻甲气化占 52 0 % ,中鼻甲反向弯曲占 2 2 4 % ,Haller气房占 4 4 8% ,筛泡过度气化占 50 0 % ,钩突偏曲占 2 1 4 %。扫描层间距为 5mm ,窦口鼻道复合体显示率为 4 9 6%。层间距 3mm为 10 0 %。结论 :窦口道复合体变异与慢性鼻窦炎的发生相关 ,鼻窦HRCT冠状位扫描是显示该区域最佳影像学技术 ,对功能性鼻内窥镜手术具有指导价值。Objective: To evaluale the value of HRCT in exarning the nasal cavity.Methods:The scanning on coronal as well as transverse section was performed on 106 patients with Philips SR-7000 spiral CT unit.The thickness of slice was 3mm,and the gap was 5mm in 94 cases,3mm in 12 cases.Results:Ninty-two per cent of all patients had chronic sinusitis,among them,81.6% had deflection of nasal septum,52.0% had concha bulla,22.4% had paradoxially bent middle turbinate,44.8% had Haller cell,21.4% had deflection of uncinate process and 50.0% had large ethmoid bulla.The display rate of ostiomeatal complex was 49.6% when the scanning was perfonned with the gap of 5mm,but it was 100% when a gap of 3mm was taken.Conclusion:The relativity between variation of ostiomeatal complex and chronic sinusitis is defmite.Comnary section is the optimal scanning place for displaying the structures mentioned above and it can be used to guide the functional endoscopic sinus surgery.
