浅表淋巴结病良恶性的彩超综合评估

摘　　要：目的 :探讨超声在浅表淋巴结病良恶性鉴别诊断中的价值。方法 :对113个病人计128个淋巴结的二维超声、彩色多普勒血流显像及脉冲多普勒分析 ,这些结果均经超声引导活检和/或手术病理证实。结果 :淋巴结良恶性的鉴别 ,单项指标(L/T<2,髓质消失 ,RI>0 70)判别 ,其敏感性分别为85 7 % ,43 9%和68 2% ,特异性分别为86 7 % ,93 3%和74 5% ,准确性分别为85 9 % ,55 5 %和68 3%。若三指标联合 ,其诊断恶性淋巴结的敏感性、特异性和准确性分别为38 5% ,96 4 %和70 7 %。结论Objective :To investigate the value of ultrasound in differentiating superficial malignant from benign lymph nodes.Materials and Methods:128 lymph nodes obtained from 113 patients were evaluated by gray scale ultrasound,color Doppler flow imaging and pulsed Doppler ultrasound.These results were confirmed by sonographically guided fine needle aspiration and/or operation.Results:In differentiation malignant from benign lymphadenopathy,using single index such as L/T<2,absence of hilus or RI>0 70,the sensitivity were 85 7%,43 9% and 68 2% respectively,the specificity were 86 7%,93 3% and 74 5%,the accuracy were 85 9%,55 5% and 68 3% respectively. But uniting these three indexes the sensitivity、specificity and accuracy were 38 5%,96 4% and 70 7% respectively.Conclusion:United mutiple indexes improved differentiating diagnosing rate of superficial lymphadenopathy by ultrasound.
