PCOS合并不孕症阴道超声及生殖激素检查的临床评价

摘　　要：目的：探讨多囊卵巢综合征（PCOS）合并不孕症时，阴道超声、血清生殖激素检查及腹腔镜诊治的临床价值。方法：受检者于月经第2-4天或闭经期检测血清睾酮（T）、卵泡刺激素（FSH）、促黄体激素（LH）、雌二醇（E2）、催乳激素（PRL）及胰岛素（INS）；阴道超声诊断卵巢多囊样改变（PCO）；腹腔镜确诊PCO，并行双侧卵巢打孔术。结果：阴道超声与腹腔镜诊断PCO符合率55．17％（64／116）。LH／FSH〉2-3占37．93％（44／116）；高雄激素血症占13．79％（16／116）；高胰岛素血症占31．03％（36／116）。阴道超声PCO与高雄激素血症、LH／FSH〉2-3比较，差异有统计学意义（X^2=43．958，P=0．000；X^2=6．930，P=0．008）。结论：PCOS合并不孕症时，阴道超声PCO辅助诊断PCOS的价值优于高雄激素血症及血清LH／FSH〉2-3，是一个较敏感而可靠的指标。Objective： To explore the clinical value of transvaginal ultrasonography, serum reproductive hormone and laparoscope when diagnosis polycystic ovary syndrome （PCOS） for PCOS with infertility. Methods： Serum testosterone （T）, follicle stimulating hormone （FSH）, luteinizing hormone （LH）, estradiol （E2）, prolactin （PRL） and insulin （INS） of subjects at 2 to 4 days of the menstrual or amenorrhea were measured. Polycystic ovaries （PCO）was diagnosed via transvaginal ultrasonography. Bilateral ovarian drilling was performed via laparoscope when diagnosed as PCO. Results： The coincidence rate of transvaginal ultrasound and laparoscopic diagnosis of PCO was 55.17% （64/116） . LH/FSH 〉 2 - 3 made up 37.93% （44/116） . Hyperinsulinemia occupied 13.79% （16/116） and hyperinsulinemia was 31.03% （36/116） . Compare transvaginal ultrasonography PCO with hyperinsulinemia and LH/FSH 〉 2 - 3, the difference was statis- tically significant （X^2 = 43.958, P = 0. 000 ; X^2 = 6. 930, P = 0. 008） . Conclusion： In the diagnosis of PCOS with infertility, the transvaginal ultrasonography PCO plays a more important role than serum LH/FSH as an auxiliary diagnostic. Transvaginal uhrasonography PCO is a method which is sensitive and reliable.
