宫腔粘连术后妊娠不良预后情况的临床调查

摘　　要：目的：评价宫腔粘连术后患者再次妊娠时不良预后的发生隋况。方法：追踪随访并分析2011年1月-2013年1月郑州大学第三附属医院247例宫腔粘连术后患者再次妊娠情况及不良预后发生情况。结果：术后随访1年，103例轻度宫腔粘连患者受孕61例，稽留流产3例，晚期流产1例，胎盘异常6例；79例中度宫腔粘连患者，受孕32例，稽留流产7例，宫颈机能不全孕中期自然流产1例，胎盘异常9例；65例重度宫腔粘连患者，受孕16例，稽留流产8例，胎盘异常7例，组间比较差异均有统计学意义（P〈0．05）。结论：随着宫腔粘连程度的加重，宫腔粘连术后患者妊娠率逐渐降低，稽留流产及分娩时胎盘异常发生率逐渐增高。Objective： To evaluate the condition of adverse prognosis for pregnant patients with intrauterine adhesions postopera- tively. Methods： 247 cases of pregnancy poor prognosis after intrauterine adhesions operation between January 2011 and January 2013 in the third affiliated hospital of Zhengzhou University were followed - up and analyzed. Results： In 103 cases of patients with mild intrauterine ad- hesions after operation, 61 cases were pregnancy in the 1 year of followed up, including 3 cases of missed abortion, 1 case of late abortion and 6 cases of abnormal placenta. In 79 patients with moderate intrauterine adhesions after operation, 32 cases were pregnancy in the 1 year of followed up, including 7 cases of missed abortion, 1 case of cervical incompetence of second trimester spontaneous abortion, 9 cases of abnormal placenta. There were 65 cases of severe intrauterine adhesions after operation, 16 cases were pregnancy in the 1 year of followed up, including 8 cases of missed abortion, 7 cases of abnormal placenta. There were statistically significant differences between groups （ P 〈 0. 05 ） . Conclusion ： With the increasing of the degree of intrauterine adhesions, the patients with postoperative pregnancy rates are gradually decreased, missed abortion and childbirth placental abnormality rates are gradually increased.
