摘  要:目的探讨子宫腺肌病痛经患者血清与病灶中神经生长因子(NGF)的表达及其与痛经的相关性,研究血清中NGF水平的临床应用价值。方法选择2010年1月至12月在复旦大学附属妇产科医院就诊并行全子宫切除的子宫腺肌病痛经患者为观察组,共25例;宫颈上皮内瘤变(CIN)3级无生育要求行全子宫切除者为对照组25例,分别取其子宫腺肌病病灶、在位子宫内膜及术前血清,并予以疼痛评分;用免疫组化法比较病灶中及子宫内膜中NGF的表达强度,并分析病灶中NGF的表达与痛经严重程度的相关性;用ELISA法测其两组血清中NGF的水平,分析患者血清中NGF的水平与痛经严重程度的相关性。结果观察组病灶中NGF的表达明显高于在位内膜(P=0.0312),且与痛经的严重程度有相关性(r=0.3964,P=0.0498);在位内膜中NGF的表达明显高于对照组子宫内膜(P=0.0143),观察组血清中NGF的表达水平明显高于对照组(P=0.0322),同时与病灶中NGF的表达强度有相关性(r=0.4126,P=0.0404),但与痛经的严重程度的相关性比较差异无统计学意义(r=0.3892,P=0.0545)。结论子宫腺肌病痛经患者血清NGF水平与子宫腺肌病的发生有关,与组织NGF的表达有相关性,其临床意义值得进一步研究。Objective The clinical value of NGF in serum has been investigated through researching the NGF in serum and the local lesions in adenomyosis with dysmenorrhea and analyzing the relationship among the serum NGF, the local lesions NGF and the degree of dysmenorrhea. Methods Twenty five patients who had adenomyosis and underwent hyster- ectomy were chosen as experimental group, and the same number of patients who had Cervical intraepithelial neoplasia m and underwent hysterectomy were chosen as control group. The serums were collected from experimental group and control group before the surgery. Eetopic endometrium from experimental group and eutopic endometrium from control group were collected in the surgery. The degree of dysmenorrhea, was detected by Visual rating scale (VAS). The expression of NGF in the local lesions was examined by immunohistochemistry. The expression of NGF in serum was examined by ELISA, which was used to analysis the relationship of the expression of NGF and the dysmenorrhea severity. Results The expres- sion of NGF in the local lesions was higher in adenomyosis pain group than the eutopic ( P = 0. 0312), the eutopic was higher in adenomyosis pain group than the control group( P = 0. 0143 ). The expression of NGF was correlated with the degree of dysmenorrhea(r = 0. 3964, P = 0. 0498 ). The expression of NGF in serum was higher in adenomyosis pain group than the control group( P = 0. 0322), and the expression of NGF in serum was not correlated with the degree of dys- menorrhea(r = 0. 3892,P = 0. 0545). However, the expression of NGF in the local lesions was correlated with the NGF in serum(r =0. 4126,P = 0. 0404). Conclusion The expression of NGF in adenomyosis was associated with the occur- rence of adenomyosis, and was correlated with the expression of NGF in the local lesions. Therefore, its clinical value is worthy for the further study.
