摘　　要：目的调查医务人员手卫生依从性与耐甲氧西林金黄色葡萄球菌(MRSA)医院感染的相关性,确定加强医务人员手卫生可有效降低MRSA医院感染的发生,为采取有效措施降低MRSA感染率提供科学的依据。方法对2011年1月-2012年6月在医院重症监护病房发生MRSA医院感染的患者进行调查,了解其病原菌耐药性,同时将上述资料与同期医务人员手卫生执行率进行比较,分析在强化手卫生措施前后,检出的MRSA株数以及MRSA感染率的临床变化情况。结果 2011年第一季度医务人员手卫生执行率为46.7%,同期检出MRSA 74株,阳性率67.9%,MRSA感染率41.6%;采取干预措施后至2012年第二季度,手卫生执行率已达82.4%,该时期检出MRSA仅12株,阳性率16.9%,MRSA感染率5.7%,差异有统计学意义。结论加强医务人员手卫生管理可阻断MRSA的传播,对控制MRSA的医院感染切实有效,并且只有加强教育和监督才能提高医务人员的手卫生依从性。OBJECTIVE To investigate the relationship between nosocomial infections caused by Methicillin-resistant Staphylococcus aureus （MRSA） and compliance of hand hygiene in medical personnel. We tried to assess the effect of intensive hand hygiene on reducing the incidence of nosocomial MRSA, so as to provide a scientific basis for tak- ing effective measures to reduce nosocomial MRSA infection rates. METHODS The inpatients with MRSA infec- tions in intensive care unit from Jan, 2011 to Jun, 2012 were investigated to understand the situation of pathogen resistance. Meanwhile, their data were compared to the rate Of executing hand hygiene of medical personnel in or- der to analyze the strains of MRSA and the incidence of MRSA infections before and after hand hygiene was strengthened. RESULTS In the first quarter of 2011, the rate of executing hand hygiene was 46.7%, and MRSA i- solated was 74 strains with the resistant rate of MRSA was 67.9% and the incidence of MRSA infection was 41.6%. After taken the intervening measures until the second quarter of 2012, the rate of executing hand hygiene was 82.4%, only 12 strain of MRSA isolated with the resistant rate of MRSA was 16.9% and the incidence oI MRSA infection was 5.7%. CONCLUSION Analysis shows that nosocomial infection of MRSA can be decreased effectively by strengthening hand hygiene management in medical personnel. Only by education and monitoring can the compliance of hand hygiene of the medical personnel be strengthened.
