摘 要:目的观察早期肠内营养对烧伤大鼠肠道能量代谢的影响并探讨其机制。方法采用30%体表面积Ⅲ度烧伤大鼠模型,将88只Wistar大鼠随机分为正常对照(C)组,早期肠内营养(EEN)组和延迟肠内营养(DEN)组。观察各组大鼠肠黏膜能量代谢的变化以及肠上皮细胞线粒体呼吸控制率(RCR)、磷氧比(P/O)及肠道氧摄取率(Oext)的变化。结果烧伤后两组大鼠肠黏膜AMP含量较伤前显著升高(P<0.01),而ATP、ADP、能荷、RCR、P/O以及Oext显著降低(P<0.05)。两组相比,EEN组AMP含量显著低于DEN组(P<0.05),而ATP、ADP、能荷、RCR、P/O以及Oext则明显高于DEN组(P<0.05)。结论早期肠内营养能够有效增加氧摄取,改善肠黏膜组织氧合,减轻线粒体呼吸功能抑制,增强有氧代谢和ATP生成。Objective To observe the effect of early enteral nutrition (EEN) on intestinal energy metabolism on burned rats and to explore its mechanisms. Methods Wistar rats were inflicted with 30% total body surface area full thickness burns and randomly divided into three groups: bum control (C), early enteral nutrition (EEN) and enteral nutrition (DEN) group. The following indices including respiratory control rate (RCR), P/O ratio, oxygen extraction (Oext) and intestinal energy metabolism were measured. Results After bum injury, in EEN and DEN groups, the contents of AMP were significantly increased, but the contents of ADP and AMP, energy charge, RCR, P/O and Oext were significantly decreased. Compare with DEN group, all indices were remarked increased except the contents of AMP in EEN group. Conclusion Early enteral nutrition can increase oxygen intake, improve intestinal mucosal tissue oxygenation effectively, lessen the degree of mitochondrial respiratory disfimction, and strengthen aerobic metabolism and ATP production.
