坐位腰椎旋转手法治疗退行性腰椎滑脱症的临床观察

摘　　要：目的观察坐位腰椎旋转手法治疗退行性腰椎滑脱症的临床疗效。方法将40例退行性腰椎滑脱患者随机分为治疗组与对照组各20例,治疗组采用坐位腰椎旋转手法+腰椎康复操,对照组采用仰卧位腰椎牵引法+腰围制动。治疗组治疗持续3周,隔日治疗1次,每周3次,共治疗9次;对照组治疗持续3周,每日治疗1次,每周5次,共治疗15次。分别于治疗前、后及疗程结束后1个月3个时间点进行疗效观察。结果治疗后治疗组总有效率95.00%,对照组85.00%,2组间差异有统计学意义(P<0.05);疗程结束后1个月,治疗组总有效率为95.00%,对照组总有效率75.00%,2组间差异有统计学意义(P<0.05);与治疗前比较,治疗后与疗程结束后1个月2组患者症状、体征及临床疼痛均有明显改善,差异有统计学意义(P<0.01)。结论坐位腰椎旋转手法治疗退行性腰椎滑脱症具有良好的短期疗效且操作安全。Objective To observe the clinical efficacy of sitting-position lumbar-vertebrae rotatory manipulation on treatment of degenerative lumbar spondylolisthesis. Methods Forty cases of degenerative lumbar spondylolisthesis were randomly divided into a treatment group and a control group, 20 cases in each one. The sitting-position lumbar-vertebrae rotatory manipulation and rehabilitation exewise were applied in the treatment group, once every other day, three times a week for continuous three weeks, totally nine times. The supine-positionlumbar-vertebraetraction and lumbar immobilization were apphed in thecontrol group, once a day, five times a week for continuous three weeks, totally fifteen times. The efficacy was observed before the treatment, after the treatment and one month after the treatment. Results After the treatment, the total effective rate was 95.00 % in the treatment and 85.00 % in the control group, which had statistical significance （P 〈0.05）.One month after the treatment, the total effective rate was 95.00 % in the treatment and 75.00 % in the control group, which had statistical significance （P 〈0.05）. Compared before the treatment, symptoms, signs and clinical pain in two groups were significantly improved after the treatment and one month after the treatment （P 〈0.01）.Conclusion The sitting-position lumbar-vertebrae rotatory manipulation has superior short-term efficacy on treatment of degenerative lumbar spondylolisthesis, which is also safe.
