推拿手法缓解颞下颌关节紊乱综合征疼痛的疗效评价

摘　　要：目的观察推拿手法对风寒湿痹型颞下颌关节紊乱综合征的镇痛效果和安全性。方法选取60例颞下颌关节紊乱患者,随机分为2组。治疗组31例采用推拿手法治疗,施推法和拇指点按法于阿是穴、耳禾髎、颊车、上关和下关穴,施拿法于外关合谷穴;对照组29例以超短波理疗。2组均每周治疗3次,2周为1个疗程。疗程结束后采用视觉模拟评分法进行疗效评定。结果 2组患者治疗后疼痛症状均有减轻,差异有统计学意义(P<0.05);治疗组疼痛评分和综合疗效优于对照组,差异亦有统计学意义(P<0.05)。2组均未发生明显不良反应。结论推拿手法可以安全有效地缓解颞下颌关节紊乱引发的头面部疼痛。Objective To observe the analgesic effect and safety of tuina manipulation on temporomandibular disorders with syndrome of obstruction of wind-cold-dampness.Methods Sixty cases of temporomandibular disorders were selected and randomly divided into two groups. The treatment group （31 cases） was treated with tuina manipulation, in which pushing and point-and-click of thumb technology were applied at shi point, Erheliao （SJ 22）, Xiaguan （ST 7）, Shangguan （GB 3） and Jiache （ST 6） and grasping technology was applied at Hegu （LI 4） and Waiguan （SJ 5）. The control group （29 cases） was treated with ultrashort wave therapy. The treatment was given three times a week in both group, and two weeks were taken as a session. After the treatment visual analogue scale （VAS） was applied to evaluate the therapeutic effects.Results After the treatment the pain was relieved in both groups, which was statistically significant （P 〈0.05）. The VAS and comprehensive efficacy of treatment group was significantly superior to those of control group （P 〈0.05）. Also there was no obvious adverse effect in both groups. Conclusion The tuina manipulation could safely and effectively relieve the pain in the face and head that is caused by temporomandibular disorders.
