单侧椎弓根螺钉固定联合TLIF技术治疗腰椎间盘突出症疗效分析

摘　　要：目的 总结单侧椎弓根螺钉固定联合经椎间孔椎体间融合术（TLIF）技术治疗腰椎间盘突出症的效果.方法 对2010年1月-2011年12月期间经单侧椎弓根螺钉联合TLIF技术治疗的17例病例进行回顾性分析,L3-L4病变2例,L4-L5病变7例,L5-S1病变8例;L4-L5合并 L5-S1病变3例.结果 全部患者均获得随访,随访时间10～30个月,平均（11.6±0.8）个月,优良率94.1%.结论 单侧椎弓根螺钉固定联合TLIF技术治疗腰椎间盘突出症具有创伤小、神经根减压彻底、术后恢复快、并发症少、复发率低、费用低等特点.严格掌握手术适应证和仔细操作是手术成功的关键.Objective To summarize the efficacy of unilateral pedicle screw fixation combined with TLIF on prolapse of lumbar interver- tebral disc. Methods Retrospective analysis was performed on 17 cases treated with unilateral pedicle screw fixation combined with TLIF between Jan 2010 and Dec 2011.2 cases had disc herniation at L3-L4,7 cases at IA-IS, 8 cases at LS-S1, and 3 cases had disc herniation at both L4-L5 and I5-S1. Results All patients received follow-up, with the follow-up period of 10 - 30 months. The median follow-up period was （ 11.6± 0.8） months. The effective rate reached 94.1%. Conclusion Unilateral pedicle screw fixation combined with TLIF on the treatment of prolapse of lumbar intervertebral disc has the merits of small injuries, completely nerve root decompression, rapid postoperative recovery, fewer complications, low recurrence rate, and low cost. Choosing the appropriate indications and operation carefully is the key to obtain a successful surgery.
