冠状动脉介入治疗对冠心病患者血浆MMP-9和IL-6水平的影响

摘　　要：目的测定冠心病患者冠状动脉介入治疗前后血浆基质金属蛋白酶-9(MMP-9)及白细胞介素-6(IL-6)的血浆浓度,分析冠状动脉支架术对冠心病患者炎症指标的影响。方法连续入选经冠状动脉介入治疗单支病变的冠心病患者55例,30例经冠状动脉造影证实冠状动脉正常的人作为对照组。采用酶联免疫吸附法检测入选患者冠状动脉介入治疗前后MMP-9和IL-6水平。结果冠状动脉介入组术后血浆MMP-9为(19.87±5.31)mg/L,显著高于术前的(15.71±5.23)mg/L(P<0.01),而对照组冠脉造影术后MMP-9为(13.78±5.58)mg/L,与术前(12.42±5.39)mg/L相比差异无统计学意义(P>0.05)。冠状动脉介入组病人术后血浆IL-6为(22.69±5.97)ng/L,显著高于术前的(19.52±5.72)ng/L(P<0.01),而对照组冠状动脉造影术后IL-6为(17.53±5.51)ng/L,与术前(16.69±5.42)ng/L相比差异无统计学意义(P>0.05)。结论冠脉介入治疗促进冠心病患者血浆MMP-9及IL-6水平的升高,是否为冠脉介入治疗术后支架内再狭窄的重要机制之一尚待进一步考证。Objective To explore the effect of matrix metalloprotelnase- 9 （MMP - 9） and interlekin - 6 （IL- 6）levels following pereutaneous coronary interventional procedures （PCI） in patients with coronary artery disease （CAD）. Methods This study involved 55 patients undergoing PCI procedures for CAD with a single coronary artery. Thirty healthy subjects with normal coronary angiography were selected as the control group. Before and after PCI or coronary angiography,plasma MMP - 9 and IL - 6 were measured by enzyme - linked immunosorhant assay. Results The plasma MMP - 9 level was significantly elevated after PCI （19.87 mg/L± 5.31 ng/L vs the preoperative level of 15. 71mg/L± 5. 23 rag/L, P〈0. 01）. The MMP - 9 level in the control group was significantly changed after coronary angiography （13.78 mg/L±5.58 mg/L vs the preoperative level of 12.42 mg/L±5.39 mg/L,P〉0.05）. PCI procedures also resulted in significant elevation of plasma IL- 6 level in the CAD patients （22.69 ng/L±5.97 ng/L vs the preoperative level of 19.52 ng/L±5.72 ng/L, P〈0.01）, but in the control group, IL - 6 showed no significant changes after coronary angiography （17.53 ng/L±5.51 ng/L vs the preoperative level of 16.69 ng/L±5.42 ng/L,P〉0.05）. Conclusion Plasma MMP - 9 and IL - 6 were elevated in CAD patients following PCI procedures.
