小儿EB病毒性肺炎细胞因子水平变化的研究

摘　　要：目的：了解细胞因子IL-2、IL-8、IL-10、INF-γ在小儿EB病毒（EBV）性肺炎的变化情况，探讨其临床意义。方法：我院2012年1～12月临床及实验室检查诊断为EBV肺炎的患儿32例为观察组，年龄1～8岁，平均（4．17±1．43）岁；同期儿童保健科门诊健康体检儿童30例为正常对照组，年龄2～7岁，平均（4．77±1．83）岁。两组受试者分别进行血清IL-2、IL-8、IL-10、INF-γ的检测，并分别比较这四种细胞因子在两组受试者中的差异。结果：与对照组比较，观察组血清IL-2、INF-γ水平降低（P〈0．01），而IL-8、IL-10水平升高（P〈0．05）。结论：小儿EBV肺炎其血清中相关细胞因子分泌紊乱，主要表现为Th1／Th2细胞平衡失调，提示机体存在相应的免疫功能障碍；对EBV感染性肺炎患儿及时进行细胞因子IL-2、IL-8、IL-10、IFN-γ等检测，对于指导治疗、判断预后具有重要的临床意义。Objective： To investigate the changes of cytokines IL-2, IL-8, IL-10 and INF-γ in children with Epstein-Barr virus （EBV） pneumonia and explore its clinical value. Methods： Thirty-two cases of children with EBV pneumonia at the age of 1 to 8 （4.17±1.43） years diagnosed by clinical and laboratory examination from January to December in 2012 in our hospital were taken as an observation group, and 30 healthy children from Child Health Care Clinic aged 2 to 7 （4.77 ±1.83 ） years were taken as a normal control group, The serum levels of cytokines IL-2, IL-8, IL-10 and INF-γ were tested respectively. The differences were compared between the observation and the control group. Results ： Compared with the control group, the levels of IL-2 and IFN-γ were decreased markedly （ P〈0.01 ）, while the levels of IL-8 and IL-10 were increased markedly （P〈0.05） in the observation group. Conclusions： Disorders of associated cytokines secretion occurred in children with EBV pneumonia, suggesting that there was a corresponding immune dysfunction, especially on Th1/Th2 cells imbalance. IL-2, IL-8, IL-10 and INF-γ need to be monitored timely in children with EBV pneumonia. It is very important for clinical guide treatment and prognosis prediction.
