摘 要:目的 研究大鼠心脏在急性全脑缺血再灌注时的病理改变及脑组织凋亡蛋白天冬氨酸特异酶切的半胱氨酸蛋白酶(Caspases)的表达情况.方法 健康雌性SD大鼠50只(180~220 g)随机分为5组:假手术组(Sham,n=10):只暴露血管而不夹闭;缺血再灌注组(I/R组,n=10):实验选用大鼠构建动物模型,以二血管阻断加低血压法,建立急性全脑缺血再灌注模型,缺血时间持续15 min,之后解除双侧颈总动脉的夹闭进入再灌注期;δ阿片受体激动剂脑啡肽乙酸酯(DADLE)处理组分为2 mg/kg组(A组,n=10)、DADLE 3 mg/kg组(B组,n=10)、DADLE 5 mg/kg组(C组,n=10):在I/R组的基础上于再灌注前经颈外静脉注射DADLE.灌注120 min后开胸取心脏,Masson特殊染色,开颅取大脑采用western blot技术检测caspase-3的含量.结果 MASSON染色结果显示,I/R组可见部分的绿色胶原纤维,排列紊乱、疏松;而Sham组未见绿色胶原纤维,细胞排列整齐、紧密、无萎缩;DADLE处理组间无显著差异,未见绿色胶原纤维,心肌细胞排列较I/R组整齐、致密.Sham组Caspase-3蛋白表达显著低于其他各组(P<0.05),DADLE处理组与I/R相比,Caspase-3表达显著降低(P<0.05),其中B、C组Caspase-3表达显著低于A组(P<0.05),B、C两组间无显著性差异(P<0.05).结论 大鼠急性全脑缺血再灌注时,脑组织凋亡蛋白Caspase-3表达水平上调同时伴有心肌细胞不同程度损害;而通过施加DADLE可以有效地减缓心、脑的缺血缺氧损伤,对器官发挥保护作用.Objective To study the pathological changes and apoptosis protein caspase - 3 expression of heart during acute cerebral ische- mia reperfusion. Methods 50 healthy female SD rats ( 180 -220g ) were randomly divided into 5 groups : sham operation group (sham, n = 10 ) : only exposed blood vessels without clipping; ischemia reperfusion group ( I/R, n = 10 ) : a modified two vessel occlusion with hypotension method to establish the model of cerebral isehemia reperfusion, 15rain after ischemia given reperfusion; DADLE treatment group, which were di- vided into three groups such as 2 mg/kg ( A, n = 10 ), DADLE 3 mg/kg group ( B, n = 10 ), DADLE 5 mg/kg group ( C, n = 10 ). DADLE were injected via external jugular vein in the I/R group on the basis of the prior to reperfusion. Perfusion 120 min after thoracotomy for cardiac, Masson special staining and craniotomy for brain using Western blot technology to detect the content of easpase - 3. Results MASSON staining showed the visible part of the green collagen fibers arranged in disorder, osteoporosis in I/R group. No green collagen fibers in the sham group. Cells arranged in rows without atrophy. There was no significant difference between DADLE group, no green collagen fibers. Myocardial ceils ar- ranged in a neat, comparing with I/R group. Expression of Caspase - 3 protein in sham group was significantly lower than that of other groups ( P 〈0.05 ). Caspase -3 expression in DADLE group was significantly lower ( P 〈0.05 ) eompared with I/R group. Caspase -3 expression of B and C group was lower than that in A group ( P 〈 0.05 ). There was no significant difference ( P 〉 0.05 ). Conclusion Acute global cerebral isehemia reperfusion, brain tissue apoptosis protein Caspase - 3 expression level increased with different degree of myocardial cell damage. The ap- plication of DADLE can effectively slow down the heart, brain ischemia hypoxia injury to organs, to play a protective role.
