血管性痴呆住院患者肺部感染的危险因素分析

摘　　要：目的探讨血管性痴呆(VD)住院患者肺部感染的危险因素,为其临床防治提供可参考依据。方法选取30例神经内科的血管性痴呆患者临床资料,对发生肺部感染患者应用多因素回归分析进行多因素分析。结果 30例VD住院患者中有5例患者发生肺部感染,感染率为16.7%;单因素分析结果显示,住院时间、慢性阻塞性肺疾病、糖尿病、低蛋白血症、长期卧床、侵入性操作、住院时间是VD住院患者发生肺部感染的危险因素(P<0.05);多因素分析结果显示,住院时间、糖尿病史、低蛋白血症、长期卧床是VD住院患者发生肺部感染的独立危险因素(P<0.05)。结论 VD住院患者发生肺部感染与住院时间、糖尿病史、低蛋白血症、长期卧床存在密切的相关性,对上述因素进行必要的干预有重要的临床意义。OBJECTIVE To discuss the risk factors for pulmonary infections in the hospitalized patients with vascular dementia（VD）so as to guide the clinical prevention and treatment.METHODS A total of 30cases of VD patients who were hospitalized the neurology department were enrolled in the study and randomly divided into the infection group and the non-infection group,then the multivariate regression analysis was performed.RESULTS Of totally 30cases of hospitalized patients with VD,the pulmonary infections occurred in 5cases with the infection rate of 16.7%.The result of univariate analysis indicated that the hospitalization duration,chronic obstructive pulmonary disease,diabetes,hypoalbuminemia,long bedridden,and invasive operation were the risk factors for the pulmonary infections in the VD patients（P0.05）;the result of multivariate showed that the hospitalization duration,diabetes history,hypoalbuminemia,and long bedridden were the independent risk factors for the pulmonary infections in the VD patients（P0.05）.CONCLUSION The incidence of pulmonary infections in the hospitalized patients with VD is related to the hospitalization duration,diabetes history,hypoalbuminemia,and long bedridden;it is of great significance to implement necessary interventions for the risk factors.
