深部真菌感染的快速检测-G试验

摘　　要：目的探讨血浆1-3-β-D-葡聚糖检测与传统真菌培养之间的关系。方法应用MB-80微生物动态快速检测系统及GKT-5MSet动态真菌检测试剂盒定量检测血浆中1-3-β-D-葡聚糖的含量,并与真菌培养结果进行比较。结果 80例高危患者1-3-β-D-葡聚糖含量增高23例,占28.75%,1-3-β-D-葡聚糖含量正常57例,占71.25%;1-3-β-D-葡聚糖含量增高组真菌培养阳性12例,占52.17%,1-3-β-D-葡聚糖含量正常组真菌培养阳性2例,占3.5%,1-3-β-D-葡聚糖含量增高组真菌培养阳性率显著高于1-3-β-D-葡聚糖含量正常组,两组差异有统计学意义(P<0.05);以真菌培养为标准,G试验的敏感度为85.71%,特异度为83.33%,阳性预测值为52.17%,阴性预测值为96.49%。结论 G试验较传统的真菌培养法快速、阳性率高,可用于深部真菌感染的早期诊断。OBJECTIVE To explore the relationship between the plasma 1-3-β-D-glucan assay and the conventional fungal culture.METHODS The concentration of plasma 1-3-β-D-glucan was measured with the use of Microbiology Kinetic Rapid Reader MB-80and Kinetic Mycetes Detection Kit GKT-5MSet,then the result was compared with the fungal culture result.RESULTS Of totally 80cases of high risk patients,the content of plasma 1-3-β-D-glucan was elevated in 23cases（28.75%）,there were 57（71.25%）cases for whom the content of plasma 1-3-β-D-glucan was normal.Among the patients with the content of plasma 1-3-β-D-glucan elevated,12cases were positive for fungal culture,accounting for 52.17%;among the patients with the content of plasma 1-3-β-D-glucan normal,2 cases were positive for the fungal culture,accounting for 3.5%,the positive rate of the fungal culture was significantly higher in the elevated content of plasma 1-3-β-D-glucan group than in the normal content of plasma 1-3-β-Dglucan group,the difference between the two groups was significant（P0.05）.With the fungal culture regarded as the standard,the sensitivity of the G-test was 85.71%,the specificity 83.33%,the positive prediction value 52.17%,the negative prediction value 96.49%.CONCLUSIONAs compared with the conventional fungal culture, the G-test is rapid with higher positive rate,which can be applied in the early diagnosis of deep fungal infections.
