摘 要:目的调查多重耐药鲍曼不动杆菌中β内酰胺类耐药基因的流行状况。方法收集2012年8月至2013年2月江苏大学附属医院和镇江市第一人民医院住院患者标本中分离得到的多重耐药鲍曼不动杆菌菌株。应用K-B纸片扩散法检测多重耐药鲍曼不动杆菌对抗菌药物的敏感性。应用PCR法检测β内酰胺类耐药基因。结果 47株多重耐药鲍曼不动杆菌除对头孢哌酮-舒巴坦,米诺环素和氨苄西林-舒巴坦的耐药率相对较低外,对其他抗菌药物的耐药率均较高。β内酰胺类耐药基因TEM、ADC、OXA-23和OXA-51的阳性率分别为91.5%(43株)、93.6%(44株)、93.6%(44株)和95.7%(45株)。结论多重耐药鲍曼不动杆菌对β内酰胺类抗生素耐药可能与β内酰胺类耐药基因的表达相关。Objective To investigate the prevalence of β-laetamase genes in multidrug resistant strains of Acinetobacter bauman-nil (MDR-AB) in Zhenjiang. Methods Multidrug resistant A. baumannii were collected from August 2012 to February 2013 from patients in Affiliated Hospital of Jiangsu University and Zheniiang First People's Hospital. Kirby-Bauer diffusion method was used to determine the susceptibilities of the strains to antimicrobial agents. PCR was performed to detect the β-lactamase genes. Results The susceptibility testing of 47 strains of multidrug-resistant A. baumannii showed relatively higher susceptibil-ity to cefoperazone-sulbactam, minocycline and ampicillin-sulbactam. However, for most of the antimicrobial agents tested, the multidrug-resistant A. baumannii were highly resistant. The prevalence of 13 lactamase genes TEM, ADC, OXA-23 and OXA-51 was 91.5% (43), 93.6% (44), 93.6% (44) and 95.7% (45), respectively. Conclusions The resistance to 13-1actam antibi-otics of multidrug-resistant A. baumannii strains may be associated with the expression of β-lactamase genes.
