分段潜行移植胸锁乳突肌双头单蒂肌瓣修复半侧颜面萎缩临床疗效分析

摘　　要：目的探讨分段潜行移植胸锁乳突肌胸骨头及锁骨头双头单蒂肌瓣对半侧颜面萎缩的修复效果。方法我科采用分段潜行移植胸锁乳突肌双头带蒂肌瓣修复5例半侧颜面萎缩患者,观察术后伤口愈合及颈部功能情况,术后随访半年以上。结果 5例移植肌瓣存活良好,颜面部萎缩畸形均得到良好矫正,颈部功能活动无异常。结论通过分段潜行胸锁乳突肌胸骨头及锁骨头双头带蒂肌瓣移位术,既达到了半侧颜面萎缩凹陷畸形的有效整复,又不会影响颈部外观及功能,明显改善患者生存质量,符合功能外科修复的发展要求,是一种临床应用效果比较理想的方法,值得推广。Objective To explore the repair effect of a new method using piecewise stealth transplantation of double heads （ sternal and clavicular） -single pedicle muscle flap of the sternocleidomastoid for progressive hemifacial atrophy. Methods Five cases of patients with the progressive hemifacial atrophy were repaired with a piecewise stealth transplantation of double heads-single pedicle muscle flap of the sternocleidomastoid at our department. Postoperative wound healing and neck function were observed, and the patients were followed-up for more than half a year. Results In all of five patients, the transplanted muscle flaps were survived well. Facial at- rophy deformity was corrected well, and there was no abnormal of neck function activities. Conclusion The new method can repair the hemifacial atrophy without effect on the neck appearance and function that obviously improves the living quality of patients. It meets the requirements of functional surgical repair and is a method with more ideal clinical application effect,which was worth of promotion.
