闭环靶控输注顺式阿曲库铵在骨科手术麻醉中的应用效果观察

摘　　要：目的探讨顺式阿曲库铵经闭环肌松靶控输注系统给药用于骨科手术的效果。方法选择60例ASAI^III级骨科择期手术患者,按随机数字表法分为闭环组与对照组各30例。闭环组经闭环肌松靶控输注系统给药,对照组在肌松监测下间断静脉推注给药。记录比较两组肌松药起效时间、肌松恢复时间、顺式阿曲库铵使用总量等指标。结果两组诱导和维持期间均能满足插管和术中肌松条件。闭环组肌松停药至TOF恢复到0.2、0.5、0.7、0.9时间均短于对照组(P<0.05;停药至拔管时间较对照组明显缩短(P<0.05)。两组诱导和维持肌松药用量差异无统计学意义(P>0.05)。结论闭环靶控输注顺式阿曲库铵用于骨科手术中能达到满意肌松效果,且术后肌松恢复比间断静脉推注更快。Objective To investigate the effect of close-loop muscle relaxant injection system using cis-atracurium in ortho- pedics surgery. Methods Sixty patients receiving selective orthopedics surgery were enrolled in the study, and randomly divided into close-loop and control group. The close-loop group was injected continuously by CLMRIS while the control group was injected intermit- tently according to the result of neuromuscular monitoring. The value of the on-set time, recovery time and total dosage of cis-atracuri- um were recorded. Results The both groups could meet the intubation and muscle relaxation conditions during induction and mainte- nance period. However, the recovery time of the close-loop group from drug discontinuance to TOFR 0. 2,0. 5,0. 7 and O. 9 were shorter than that of the control group （P 〈 0. 05 ）. The time of the close-loop group from drug discontinuance to extubation was also shorter than that of the control group （P 〈 0. 05）. Total dosage of cis-artracurium had no significant difference between the two groups. Con- clusion The close-loop muscle relaxant injection system using cis-atracurium can achieve satisfactory muscle relaxation in orthope- dics surgery with a faster recovery than the intermittently infusion.
