膝关节单髁置换术治疗膝关节单间室退变的24例临床观察

摘　　要：目的探讨膝关节单髁置换术（UKA）的技术难点，观察UKA治疗膝关节单间室退变的临床效果。方法回顾性分析2010年7月-2015年9月收治的24例膝关节内侧间室退变患者，24例（24膝）均获得2年以上随访。左僻8膝，右侧16膝，均为内侧间室退行性变，骨性关节炎。依照AHLBACK标准：Ⅱ期9例，Ⅲ期15例。采用髌内侧旁小切口行uKA治疗。膝关节HSS评分（包括关节活动度、疼痛、关节功能评价）。告知患者来本院复查拍片，进行长期随访和总结。结果术后2周拆线，切口I期／甲级愈合。出院后1～6个月随访有7例出现膝关节内侧疼痛，行保守治疗，半年后复查疼痛症状消失。24例患者随访18～32个月，无假体周围感染、假体松动、剩余间室退变。HSS评分和术前相比差异有统计学意义（P〈0.05）。结论严格把握手术适应证，通过UKA治疗膝关节退变性骨关节炎的早期疗效可靠UKA的近中期疗效可与全膝关节置换相媲美，而且UKA具有创伤小、出血少、术后恢复快的优点。Objective To explore the surgery surgical technical difficulties of unicondylar knee replacement, observe the early efficacy of unicondylar knee arthroplasty for single compartmental knee disease. Methods 24 cases patients were performed by unicondylar knee arthroplasty with single compartmental knee disease in the hospital from 2010 to 2013. Clinical symptoms are characterized by loading the medial compartment pain and tendemess.AHLBACK standard： I] period 9 cases, IIIperiod 15 cases .Eliminate surgical contraindications, mediopatellar beside the small incision line UKA treatment. Knee joint HSS score （including joint mobility, pain, joint function assessment）, and statistical analysis. All patients were followed up, none of the patients lost to follow-up,Results Take out stitches after 2 weeks, stage I incision/class a healing. 1-6 months follow- up after discharge, there are 7 cases with knee medial pain, conservative treatment, check after half an year pain symptoms disappear.All patients were followed-up for 18-32 months.No infection around the prosthesis, prosthesis loosening, no surplus compartments degeneration.The HSS score, the maximum degree of flexion and extension were improved significantly in the latest follow-up. Conclusion With control surgical indications, Unicondylar, knee arthroplasty has a satisfing early outcome. [
