Snodgrass与Duckett术式在尿道下裂治疗中的临床应用

摘　　要：目的探讨横裁带蒂包皮岛状皮管尿道成形术(Snodgrass术)与尿道板纵切卷管尿道成形术(Duckett术)治疗尿道下裂的疗效和适应证。方法回顾性分析831例尿道下裂患儿的临床资料,比较Snodgrass术式(436例)与Duckett术式(395例)进行治疗的手术成功率及其并发症。结果患儿均获得随访,随访2～36个月。采用Snodgrass术一次手术成功395例,成功率90.6%(395/436);术后发生并发症41例(9.4%)。Duckett术一次手术成功330例,成功率83.5%(330/395);术后发生并发症65例(16.5%)。结论 Duckett与Snodgrass是治疗尿道下裂常用的术式,且效果满意,临床术式的选择应根据患儿阴茎下弯程度、尿道缺损情况及术者对术式理解掌握水平决定。Objective To investigate the application of Snodgrass and Duckett technique in clinical treatment of hypospadias. Methods The efficacy and complications for 831 cases diagnosed with hypospadias operated with Snodgrass and Duckett technique from January 2005 to february 2013 were analyzed retrospectively. Results All patients were followed up for more than 2 months. For Snodgrass technique, 395 cases were successfully operated in the first operation, the successful rate was 90.6%; 41 cases had complications. While for the Duckett technique, 330 cases were successfully operated, the successful rate was 83.5% and 65 cases had complications. Conclusion Duckett and Snodgrass technique are common surgical approaches for the treatment of hypospadias, and both have satisfied results. We should choose one of the two techniques according to the degrees of penis blend down, urethral defect, and understanding of the different surgical approaches of the operating surgeon.
