关节镜自体腘绳肌腱双束重建前交叉韧带的临床疗效观察

摘　　要：目的探索关节镜下采用自体胭绳肌腱双束重建前交叉韧带的临床疗效。方法利用关节镜对26例膝关节前交叉韧带断裂患者进行双柬重建术。术前及术后随访时行Lachman试验、前抽屉试验、抽移试验检查，术后随访12-24个月，平均18个月。比较术前与术后患者膝关节功能的Lysholm评分、Tegner评分、膝关节IKDC评分。结果术前Lachman试验阳性24例（92．3％），前抽屉实验阳性20例（76．9％），抽移试验阳性19例（73．1％）。术后所有患者I。achman试验、前抽屉实验、抽移试验均为阴性。术后与术前相比阳性率下降有统计学意义（P〈0．05）。术前I。ysholm评分（66．1±6．3）、Tegner评分（3．8±1．9）、膝关节IKDC评分（67．3±6．4）。术后Lysholm评分（95．1±2．3）、Tegner评分（6．6±1．5）、膝关节IKDC评分（92．5±3．1），术后评分与术前相比差异有统计学意义（P〈0．01）。结论自体胭绳肌腱双束重建前交叉韧带能更好地有效恢复膝关节稳定性，疗效满意。Objective To investigate the clinical effect of the Arthroscopic double-bundle reconstruction of anterior crueiate ligament with hamstring tendon autografts. Methods 26 patients with anterior crueiate ligament fracture were reconstructed by using double-bundle hamstring tendon autografts. The 12 - 24 months of preoperative and postoperative follow-up surveys were conducted and the line Lachman test, an- terior drawer test and pumping shift test were performed simuhabneously. The knee joint functions were evaluated by comparing the Lysholm scores, Tegner scores and IKDC scores. Results The preoperative L achman test result was positive in 24 patients （92.3%）, the anterior drawer test result was positive in 20 cases （76.9%） and the pumping shift test result was positive in 19 cases （73.1%）. The postoperative re- sults of Lachman test, the anterior drawer test and pumping shift test were negative in all patients. The preoperative and postoperative positive rates were significantly decreased （P 〈0.05）. The preoperative Lysholm score was 66.1 ±6.3, the Tegner score was 3.8 ±1.9 and the Knee IKDC score was 67.3 ± 6. 4, while the postoperative Lysholm score was 95.1 ± 2.3, the Tegner score was 6.6± 1.5 and the knee IKDC score was 92.5 ± 3.1, there existing significant difference before and after operation （P 〈0.01）. Conclusion The double-bundle reconstruction of anterior cruciate ligament with hamstring tendon auto- grafts can provides good dynamic stability of the knee joint, and the clinical effects were satisfactory.
