摘  要:目的探讨血小板膜糖蛋白(GP)及血小板最大聚集率(PAgT)在冠心病合并高血压患者中的临床意义。方法将116例冠心病分为稳定性心绞痛(SAP组,36例),不稳定性心绞痛(UAP组,38例),UAP并高血压病(UH组,42例),并选取48例健康者为对照组。按心绞痛发作情况将冠心病患者再分成有事件组(56例)和无事件组(60例)。测定各组总胆固醇、三酰甘油、低密度脂蛋白(LDL-C)、脂质过氧化物丙二醛(MDA)、GP、及PAgT水平,并对有事件组患者PAgT水平作相关性分析。结果四组间总胆固醇、三酰甘油、LDL-C、MDA、GP、及PAgT水平比较,差异均有统计学意义(P均<0.05)。有事件组GP、PAgT及MDA水平均较无事件组明显升高(P均<0.05),且有事件组患者PAgT水平与GP及MDA水平相关性分析发现均呈明显正相关(P均<0.05)。结论 GP、PagT升高以及脂质过氧化损伤在冠心病患者心绞痛事件发生的病理过程中起重要作用,且高血压病可以加重这一病理过程。Objective To investigate the changes and relationships of platelet membrane glycoprotein (GP) and platelet aggregation rate (PAgT) in coronary artery disease (CAD) patients with hypertension. Methods Total of 116 patients with CAD were enrolled including stable angina pectoris patients (SAP group, 36 cases), unstable angina pectoris patients (UAP group, 38 eases) and UAP patients with hypertension group (UH group, 42 eases), and 48 healthy persons served as control. The 116 patients with CDA were further divided into events group (56 cases) and no events group (60 cases) according to angina pectoris events. The levels of total cholesterol, triaeylglyeerol, low density lipoprotein cholesterol (LDL-C), lipid peroxide malondialdehydin (MDA), GP and PAgT were measured, and linear regression analysis were used for statistical analysis in patients with events group. Results The levels of total cholesterol, triacylglycerol, LDL-C, MDA and GP, PAgT were significantly different among four groups (all P〈 0.05). The levels of GP, PAgT and MDA in the events group were markedly higher than those in the no events group (all P〈 0.05). Furthermore, the PAgT in patients of the events group was correlated positively with GP and MDA (all P〈 0.05). Conclusion GP, PagT and lipid peroxide play important roles in the pathological process of angina pectoris events in CAD patients which were aggravated by hypertension.
