不同年龄段合并轻中度上呼吸道感染患儿全身麻醉的安全性评估

摘　　要：目的：评估近期合并轻中度上呼吸道感染（URI）患儿实施全身麻醉的安全性。方法：择期手术插管全身麻醉患儿207例，依据术前上感症状、体征，分为非URI组（unURI组，n=140），轻度URI组（miURI组，n=50），中度URI组（moURI组，n=17）。监测并记录手术期、术后呼吸系统相关并发症，按年龄段分层比较。结果：手术期≤3岁患儿：moURI组咳嗽、屏气的发生率增高（P〈0．05）；4～12岁：呼吸系统并发症发生率组间差异无统计学意义（P〉0．05）。术后各年龄段：moURI组多痰的发生率增高（P〈0．05）；其他并发症发生率组间差异无统计学意义（P〉0．05）。结论：合并轻度URI、年龄4～12岁的合并中度URI患儿实施气管插管全身麻醉相对安全．而年龄≤3岁合并中度URI患儿需全面评估风险利益关系后慎重实施全身麻醉。Objective To evaluate the security of general anesthesia for different age groups children with mild to moderate upper respiratory infection （URI）. Methods 207 cases of children who undergoing general anesthesia for elective surgery, were divided into non-URI group （unURI group, n = 140）, mild URI group （miURI group, n = 50）, moderate URI group （moURI group, n = 17）, according to the preoperative symptoms and signs of URI. Monitoring and recording the perioperative and postoperative respiratory-related complications, stratified according to age. Results （ 1 ）The incidence of secretions and breath holding in moURI group were increased in perioperation period ≤3 years children （P 〈 0.05）. The others showed no significant difference in in perioperation period 4 - 12 years ehildren（P 〉 0.05）. （2）Tthe incidence of secretions in moURI group were increased, during postoperative period （P 〈 0.05）.The others showed no significant difference between the three groups （P 〉 0.05）. Conclusion It is relatively safe to implemented tracheal intubation general anesthesia for the children merger with mild URI or moderate URI in 4 - 12 years children. But the ≤3 years old children with moderate URI need a comprehensive assessment between the risk and benefit relationship before general anesthesia.
