摘 要:目的观察鼻渊舒(辛夷、苍耳子、栀子、黄芪、白芷、黄芩、柴胡、薄荷、细辛、川木通、川芎)对慢性鼻-鼻窦炎模型鼻窦黏膜上皮糖皮质激素受体的受体亲和力及转录激活能力等功能的影响,从细胞抑炎机制角度,探讨鼻渊舒治疗慢性鼻-鼻窦炎的可能机制。方法选取100只健康新西兰大白兔,按每组20只,随机分为正常、模型、假手术、克拉霉素、鼻渊舒组,建立慢性鼻-鼻窦炎模型,正常、模型、假手术组不干预,克拉霉素组、鼻渊舒组分别予以克拉霉素25 mg/(kg·d)、鼻渊舒口服液1.5 mL/(kg·d)灌胃,治疗14 d后取各组鼻窦黏膜上皮,HE染色后观察鼻窦黏膜组织的病理形态学改变,受体的放射性配基结合分析及报告质粒分析法观察糖皮质激素受体的受体结合容量、解离常数及转录激活能力。结果模型组鼻窦黏膜呈慢性炎症改变,黏膜炎细胞浸润,腺体及杯状细胞明显增生;模型组黏膜上皮糖皮质激素受体解离常数较正常组显著增高(P<0.01),转染质粒pMTV-CAT后,氯霉素乙酰转移酶(CAT)表达较正常及假手术组显著下降(P<0.01)。鼻渊舒治疗后,鼻窦黏膜上皮得到较好修复,上皮组织炎细胞浸润、杯状细胞及腺体少见增生;糖皮质激素受体解离常数显著低于模型组(P<0.01),与正常组比较无显著性差异(P>0.05),转染pMTV-CAT质粒后,CAT表达显著高于模型组(P<0.01),与正常及假手术组比较无显著差异(P>0.05)。结论鼻渊舒能显著改善慢性鼻-鼻窦炎模型鼻窦黏膜慢性炎症状态,其机制与提高黏膜上皮糖皮质激素受体的受体亲和力及转录激活能力有关。AIM To investigate the influence of Biyuanshu Oral Liquid (Magnoliae Flos, Xanthii Fructus, Gardeniae Fructus , Astragali Radix, Angelicae dahuricae Radix, Scutellariae Radix, Bupleuri Radix, Methae Herba, Asari Herba, Clematidis armandii Caulis, Chuanxiong Rhizoma ) on the affinity and transcriptional activity to glucocorticoid receptors (GR) of nasal sinuses mucosal epithelial cells in rabbit with chronic rhinosinusitis (CRS). METHODS One hundred New Zealand rabbits with CRS were randomly assigned into model group, sham operation group, clarithromycin group, Biyuanshu Oral Liquid group, and normal group without CRS, 20 in each group. There were not any interventions on normal group, model group and sham operation group. Clarithromycin group was given clarithromycin 25 rag/( kg . d), and Biyuanshu Oral Liquid group was given Biyuanshu Oral Liquid 1.5 mL/( kg . d) for 14 days. After the treatment, nasal sinuses mucosal tissue was collected to observe their pathological changes after HE dyeing, and to detect the maximum binding capacity, dissociation constant (KD) and transcriptional activity of GR with radioligand binding assay and reporter plasmids analysis. RESULTS In model group, chronic inflammation and inflammatory cell infiltration, epithelial cells and hyperplasia of glandular organs and goblet cells with CRS were obvious. The dissociation constant of nasal sinuses mucosal epithelium cytoplasm GR was higher than that in the normal group ( P 〈 0.01 ). After plasmid pMTV-CAT transfection, the CAT expression was lower than that in the normal and sham operation group (P 〈 0. 01 ). After the treatment with Biyuanshu Oral Liquid, the nasal sinuses mucosa epithelium was repaired, and inflammatory cell infiltration, hy- perplasia of epithelial cells and glandular organs and goblet cells were rare.
