医院内肺部感染患者的病原菌分布及中西药联合治疗

摘　　要：目的分析医院内肺部感染患者的病原菌分布,观察中西药联合治疗的效果。方法 127例医院内肺部感染患者随机分为联合组和西医组,西医组给予左氧氟沙星,联合组给予左氧氟沙星+痰热清注射液。评价病原菌分布,比较2组的细菌学疗效和症状疗效。结果共分离出127株病原菌,包括革兰阴性菌96株,占75.59%,革兰阳性菌31株,占24.41%。分布最多的3种细菌为铜绿假单胞菌、金黄色葡萄球菌和肺炎克雷伯菌,分别占24.41%、17.32%和16.54%。联合组细菌清除率为90.63%,西医组为69.84%,2组比较差异具有统计学意义(P<0.05)。联合组总有效率为92.19%,明显高于西医组的73.02%,2组比较差异具有统计学意义(P<0.05)。结论铜绿假单胞菌是医院内肺部感染的主要致病菌,左氧氟沙星联合痰热清能明显提高临床疗效。Objective To explore the pathogenic distribution of hospital-acquired pneumonia （HAP） and investigate the effects of combined traditional Chinese and western medicine on the infections. Methods A total of 127 cases of HAP were randomly divided into combination treatment group and western medicine group. The western medicine group was treated with levofloxacin, and the combination treatment group was given Tanreqing injection additionally. The pathogenic distribution was evaluated, and the bacteriological efficacy and the total efficacy rate in improving symptom were investiga- ted and compared. Results A total of 127 strains of bacteria were isolated including 96 strains of Gram-negative bacteria and 31 strains of Gram-positive bacteria, accounted for 75.59% and 24.41%, respectively. The three most common bacte- ria were pseudomonas aeruginosa, staphylococcus aureus and Klebsiella pneumoniae, accounted for 24.41%, 17.32% and 16.54% , respectively. The bacterial clearance rate in joint group was 90.63% , significantly higher than 69.84% in west- ern group（ P 〈 0.05）. The total efficacy rate in improving symptom in joint group was 92.19% , significantly higher than 73.02% in western group （P 〈 0.05 ）. Conclusion Pseudomonas aeruginosa are the main pathogens of HAP. Levofloxa- cin combined with Tanreqing injection can obviously improve the clinical curative effect.
