摘 要:目的:了解医院门诊药房自动化实施后对患者取药等候时间的影响。方法:从某医院信息系统获取门诊药房自动化实施以来2012年10月-2013年2月处方信息(每月抽取一日),对处方特点、患者取药等候时间、自动发药机使用情况等进行分析。结果:东院区门诊药房高峰期日均处方量4 509(4 096~4 866)张,日均发放药品高达32 906(29 461~32 133)盒(或瓶),每张处方平均发放药品数量7.3(7.1~7.7)盒;患者取药等候时间从自动化实施后第1个月的11.6 min下降到第4个月的8.5 min,高峰时段患者取药等候时间从自动化实施后第1个月的16.6 min下降到第4个月的11.8 min;自动发药机参与了约90.4%(89.3%~91.0%)处方的调配工作,其中约43.8%(42.8%~45.7%)的处方完全由发药机自动调配,不需要药师调配。自动发药机发出药品数量占总发药数量的60.1%(57.6%~61.1%)。结论:自动发药机参与处方调配比例和发放药品比例是评价自动发药机的重要指标。自动发药机和预调配模式可调高处方调配速度,缩短患者取药等候时间,促使药师工作模式从"以药品为中心"转换成"以病人为中心"。OBJECTIVE To understand the influence of automated pharmacy system on waiting time in outpatient pharmacy. METHODS Information of prescriptions was sampled from Oct 2012 to Jan2013(one day per month)since automated dispensing devices were put into use on Oct 4th 2012.Prescriptions,waiting time and the performance of automated dispensing device were analyzed.RESULTS 4 509(4 096-4 866)prescriptions were dispensed and distributed per day,and 32 906(29 461-32 133)boxes or bottles of drugs were distribution per day.There was 7.3(7.1-7.7)boxes or bottles of drug per prescription.Average waiting time decreased from11.6 min(the 1st month after automated pharmacy system)to 8.5 min.(the 4th month after automated pharmacy system).The waiting time in peak hours decreased from16.6 min(the 1st month after automated pharmacy system)to 11.8 min.(the 4th month after automated pharmacy system).The automated dispensing device played role in 90.4%(89.3%-91.0%)of outpatient prescriptions,and 43.8%(42.8%-45.7%)of prescriptions were dispensed totally by automated dispensing device.60.1%(57.6%-61.1%)of drugs were dispensed by automated dispensing device,which were more than drugs dispensed by hands.CONCLUSION The automated dispensing device could shorten the waiting time in outpatient pharmacy,so pharmacists would have more time to provide pharmaceutical care for outpatients.
