整治前后体外循环心内直视术围手术期预防用抗菌药物调查分析

摘　　要：目的:对某院抗菌药物专项整治前后体外循环心内直视术围手术期抗菌药物预防应用情况(简称预防用药)进行调查,为临床围手术期合理使用抗菌药提供帮助。方法:记录该院2010年7-12月和2012年7-12月行体外循环心内直视术的全部患者信息,其中前者纳入整治前组,后者纳入整治后组。对2组患者的预防用药情况进行统计分析。结果:整治后预防用药选药合理率从4.0%上升到89.7%(P<0.01),预防用药时机合理率明显提高(0%vs 92.8%,P<0.01),预防用药疗程从(15.0±3.2)d缩短至(4.9±1.8)d(P<0.01)。但整治前后的术后感染率未见明显变化(P>0.05)。结论:抗菌药物专项整治活动,显著促进了该院体外循环心内直视术的预防用药合理性,但该项管理工作仍需制度化、长期化。OBJECTIVE To investigate the prophylactic application of antibacterials during peri-operative period of heart operation with cardiopulmonary bypass before and after intervention,helping for clinical rational use of antibacterials in peri-operation period.METHODS The informations of all patients undergoing open-heart surgery with cardiopulmonary bypass in our hospital from Jul.-Dec.2010 to Jul.-Dec.2012 were recorded.93 cases of patients（in Jul.-Dec.2010）were admitted into ‘before intervention＇group;97 cases of patients（in Jul.-Dec.2012）were admitted into‘after intervention＇group.Statistical analysis was done on the prophylactic use of drugs in patients of the 2 groups.RESULTS After intervention,the rate of rational prophylactic use of drugs were increased from4.0%to 89.7%（P0.01）.The reasonable rate of preventive medication time were significantly increased（0%vs 92.78%,P0.01）.The duration of prophylactic treatment were shortened from（15± 3.2）d to（4.9±1.8）d（P0.01）,while there was no obvious change in postoperative infection rate（P0.05）.CONCLUSION The special intervention activities for antibacterials significantly promoted the rational prophylactic use of drugs in open-heart surgery with cardiopulmonary bypass in our hospital,but the management work still needs to be institutionalized,long-term oriented.
