摘 要:目的研究HBV感染者HBV特异性及天然非特异性免疫反应。方法 ELISPOT、ELISA比较HBV感染免疫清除期与免疫耐受期患者的PBMCs,分别受HBcAg、CpG-ODN刺激分泌INFγ、INFα的差异。结果 CCpG-ODN刺激HBV感染者PBMCs,100.00%、40.00%的免疫清除期、免疫耐受期患者,IFNα分泌实验阳性,二者之间差异有统计学意义(P<0.01);HBcAg刺激HBV感染者PBMCs分泌IFNα,免疫清除期、免疫耐受期患者分别为(578.37±7.73)、(655.25±3.80)ng/L,阳性率分别为12.50%和68.81%(ELISA),差异有统计学意义(P<0.05);用ELISPOT方法,2组患者的阳性率分别为0、87.50%,差异有统计学意义(P<0.05)。结论免疫清除期、免疫耐受期HBV感染者,PBMCs受HBcAg、CpG-ODN刺激分泌INFγ、INFα的HBV特异性及天然非特异性免疫反应均有差异,鉴别二者,ELISPOT优于ELISA。Objective The difference in HBV specific and non-specific immune responses between patients with HBV infection in clearance period and those in tolerant period was studied. Methods Peripheral blood mononuclear cells (PBMCs) from patients in the clearance period and those in the tolerance period were stimulated with CpG-ODN or HBcAg. Results If comparing the expres- sion of IFN-a stimulated with culture media 1640, those stimulated with CpG-ODN were significantly higher for 100.0% of the patients in the clearance period and 40.0% of the patients in the tolerance period; there was a significant difference between them. The expression of interferon-γ (INF-γ) by PBMCs from patients in the clearance period and in the tolerance period were 578.37 ± 7.73 and 655.25 ± 3.80 ng/L, respectively; 12.5% and 68.81% of the patients in the tolerance period and in the clearance period, respectively, were tested positive by ELISA. By ELISPOT, 0 and 87.5% of the patients in the tolerance period and in the clearance period were tested positive, respectively. Conclusion The differences in HBV specific and non-specific immune re- sponses and in INF- γ and INF- a expressed by PBMCs stimulated with HBcAg and CpG-ODN were significant between the pa- tients in the clearance period and those in the tolerant period as tested by ELISPOT and ELISA. ELISPOT was better than ELISA.
