摘  要:目的了解大连地区女性生殖道人类乳头瘤病毒(Human Papillomavirus,HPV)感染的感染率、亚型分布及年龄分布情况,为HPV感染的筛查和防治提供科学指导。方法采用聚合酶链式反应(Polymerase Chain Reaction,PCR)体外扩增和DNA反向点杂交相结合的DNA芯片技术对大连地区2010年8月-2012年9月来某院就诊的2 755例成年女性的HPV感染情况进行定性检测并加以分型。结果在2 755例成年女性中,共检出21种HPV基因型,HPV阳性检出率为23.19%。其中,单纯高危型感染率为16.33%,单纯低危型感染率为3.88%,低危型与高危型混合感染率为2.98%;单一、双重、三重、四重型别感染率分别为16.70%、4.61%、1.31%和0.58%。HPV感染率中最高的高危亚型和低危亚型分别为HPV16型(5.81%)和HPV43型(4.03%)。不同年龄检查者的HPV感染率差异和其单一及多重感染构成具有统计学意义,各年龄HPV感染者均以单一及高危感染为主,但在30岁以下及60岁以上年龄HPV感染者中高低危合并多重感染高于其他年龄段。结论大连地区2 755例成年女性共检出21种HPV基因型,HPV阳性检出率为23.19%。检出的HPV感染主要为单一基因型和单纯高危基因型,高危型和低危型则分别以HPV16和HPV43检出率最高。对于不同年龄段的检查者,其HPV感染率不同,单一及多重感染的构成也有差别;同时,30岁以下及60岁以上年龄段女性的高低危合并的多重感染高于其他年龄段的女性。Objective To provide scientific basis for the prevention and control of HPV infection, HPV infection rate, subtype distri- bution and HPV infection status in women of different age in Dalian area were examined. Methods 2 755 cervical tissue specimens collected from Dalian females in August 2010 to September 2012 were examined with the combination of Polymerase Chain Reac- tion method and the DNA reverse point hybridization method. Results Out of 2 755 cervical tissue specimens, 21 HPV genotypes were detected, and the positive HPV infection rate was 23.19%o, where high-risk genotype, low-risk genotype and low-risk + high-risk genotype HPV infection rate was 16.33%, 3.88% and 2.98% respectively; single, double, triplex and quadruple genotype HPV infection rate was 16.70%, 4.61%, 1.31% and 0.58% respectively. Of all the cases, the high-risk genotype and low-risk genotype was HPV16 and HPV43, respectively. In addition, it was found that the difference in HPV infection rate and the distribu- tion of the HPV single and complex infection in examinees of different age groups were statistically significant. The predominant HPV infection in examinees of different ages was the single and high-risk genotype. The low-risk + high-risk multiple genotype HPV infection rate in the examinees younger than 30 and older than 60 was higher than other age groups. Conclusion 21 HPV genotypes and 23.19% positive HPV infection rate were detected from 2 755 cervical tissue specimens. The dominant HPV geno- types were single genotype and high-risk genotype, and the primary high-risk and low-risk genotype was HPV16 and HPV43 re- spectively. For the examinees in different age groups, HPV infection rates and the corresponding distribution were different. In addi- tion, examinees younger than 30 and older than 60 had a higher low-risk + high-risk multiple genotype HPV infection rate.
