摘 要:研究补充左旋精氨酸(L-Arg)对改善庆大霉素(GM)肾损害的作用和机理。方法:利用实验大鼠观察肾脏病理形态学变化,测定血、尿一氧化氮(NO)、肌酐(Cr)和HCO_3^-,尿糖、蛋白,和尿N-乙酰-β-D-氨基葡萄糖苷酶(NAG)动态变化。结果:补充L-Arg能减轻GM肾损害的病理形态学改变和以下生化改变:增加尿中NO水平,使尿中NAG延迟增高,但也可造成一些不利影响:诱发代谢性酸中毒,致血Cr、尿糖和尿蛋白增加。结论:L-Arg对GM肾损害有保护作用,但也产生一些不利影响。To work over the role and mechanism of L-arginine (L-Arg) supplement in improving gentamycin (GM) nephrotoxicity. Methods:By means of Wistar rats,the items of investigation were comprised of morphologic changes of kidney, concentrations of creatinine (Cr) , bicarbonate (HCO_3^- ) and metabolic products of nitric oxide (NO_x^- ) in blood and urine,urine glucose,protein and dynamic changes of N-acetyl-β-D-aminoglucosidase (NAG) ,et al. Results:After L-Arg supplement, most of morphologic changes caused by GM nephrotoxicity were ameliorated; urine NO_x^- level was increased, while serum NO_x^- level was less influenced; the delayed increase of urine NAG was noticed; but a metabolic acidosis was induced accompanied with increased serum Cr and aggravated glucosuria and proteinuria. Conclusion: Although the L-Arg has a pro-tective role in kidney damage caused by GM nephrotoxicity,it may also produce some disadvantageous effects on the body.
