超声对小儿继发性肠套叠的诊断价值

摘　　要：目的探讨小儿继发性肠套叠的超声图像特点及诊断价值。方法回顾性分析32例继发性肠套叠的患儿资料,总结不同病理诱发点的超声图像特征。结果梅克尔憩室9例,幼年性息肉7例,Peutz-Jeghers综合征息肉2例,囊肿型肠重复畸形5例,淋巴瘤4例,过敏性紫癜2例,异位胰腺1例,阑尾炎1例,肠黏连1例。超声诊断肠套叠32例,发现病理性诱发点18例,其中17例明确诊断,1例误诊;其余14例初次超声检查未发现病理诱发点,于空气灌肠复位成功后再行超声检查诊断2例梅克尔憩室,1例阑尾炎,3例肠套叠并肠坏死。结论梅克尔憩室、肠息肉及肠重复畸形是最常见的病理性诱发点。超声不仅能诊断肠套叠,而且能发现部分病理性诱发点,判断套叠肠管血运,为临床治疗提供重要依据。Objective To investigate the value of ultrasound in the diagnosis of secondary intussusception in chil- dren. Methods To retrospectively review 32 secondary intussusception cases from clinical and ultrasonic features and summarized the ultrasonic characteristics of different pathologic lead point. Results There were 9 cases as Meckel di- verticulum, 7 cases as juvenile polyps, 2 cases as Peutz-Jeghers syndrome, 5 cases as cystic intestinal duplication,4 ca- ses as intestinal lymphoma,2 cases as Henoch-Schonlein purpuras, 1 case as eetopic pancreas, 1 case as appendicitis, 1 case as intestinal adhesion. All intussusceptions were diagnosed by ultrasound and 18 pathologic lead points were found in which 17cases were specific diagnosed and 1 misdiagnosed. There were no finding with pathologic lead points in 14 intussusceptions when first ultrasound examination in which 2 Meckel diverticulum and 1 appendicitis were depicted by ultrasound after air enema. There were 3 intussusceptions complicated by intestinal necrosis. Conclusions Meckel di- verticulum,intestinal polyp and intestinal duplication are the most common pathologic lead point. The ultrasound can not only diagnose the intussusception, but also identify a part of pathologic lead point,judge the blood supply of the in- tussuscepted intestinal tubes,provide important basis for clinical treatment.
