摘 要:目的探讨动态三维对子宫输卵管超声造影通畅性的评价及临床应用价值。方法选择女性不孕症患者32例,采用动态三维对子宫输卵管超声造影技术观察输卵管形态及走行,判断输卵管通畅性。其中19例分别进行子宫输卵管超声造影和X线子宫输卵管造影,评价2种方法对于诊断输卵管通畅程度的一致性。结果 32例女性不孕症患者中4例已行单侧输卵管切除,共60条输卵管,其中36条通畅,15条通而不畅,9条阻塞。19例38条输卵管分别进行子宫输卵管超声造影和X线子宫输卵管造影结果比较,总体符合率为81.6%(31/38),一致性检验Kappa值为0.682,一致程度较好。结论动态三维子宫输卵管超声造影技术能够实时动态显示输卵管的形态及走行,简便易行,安全经济,可以作为评价输卵管通畅性的有效方法,对输卵管病变引起的不孕症的临床诊断提供可靠依据。Objective To assess the accuracy and clinical application value of the 3D-HyCoSy in evaluation of the fallopian tube patency. Methods Fallopian tube patency in 32 cases of infertile women was evaluated using 3D-HyCo- Sy. Among them, 19 patients accepted the examination of hysterosalpingography(HSG). The estimate of the fallopian tube patency in 3D-HyCoSy were compared with those in HSG in two methods. Results In our study,4 patients had only unilateral tube. 60 uterine tubes were demonstrated by 3D-HyCoSy,among which 36 tubes were diagnosed to be patent, 15 incompletely occluded, and 9 occluded. 19 patients accepted the examination of HSG. The estimate of the fal- lopian tube patency in 3D-HyCoSy were compared with those in HSG. The total accuracy was 81.6% (31/38). Com- pared with HSG,the fallopian tube patency of two methods had high consistence,the Kappa value was 0. 682. Conclu- sions 3D-HyCoSy could display the morphological character of fallopian tube and assess the patency. 3D-HyCoSy should be considered a practical,non-invasive method for evaluating tubal pateney.
