摘  要:目的探讨超声引导下冷循环微波消融治疗脾外伤的可行性及有效性。方法 16只小香猪在开腹直视下采用手术刀片切割脾脏,建立Ⅱ级脾外伤模型,随机分为治疗组和对照组,每组8只。治疗组采用冷循环微波对脾外伤灶进行消融治疗,治疗功率、时间参数选用80 W/600 s,其中6只在超声引导下治疗,2只在开腹直视下治疗。对照组不做治疗。术后观察6 h,以解剖及病理检查进行疗效评估。结果治疗组8只均1次性微波消融凝固止血成功。对照组脾周积血增多,脾包膜及脾实质呈不同程度的破裂,剖腹探查时均可见血液自破裂口外溢。结论超声引导下冷循环微波能有效治疗Ⅱ级脾外伤,有望成为一种保脾治疗脾外伤的新方法。Objective To evaluate the feasibility and efficacy of ultrasound guided cold circulation microwave ab- lation in treating splenic trauma. Methods Open the abdomen and Cut spleens of the 16 little pigs under direct vision, make splenic trauma model of Ⅱ grade, then randomly divided them into 2 groups: treatment group and control group, with 8 in each. The treatment group was treated by microwave ablation, the treatment power and time were set at 80 W/600 s, 6 cases in this group were done by ultrasound guidance, 2 were under direct vision; And the control group pigs were not treated. Postoperative observation was 6 hours, and the results evaluated by anatomy and Patho- logical examination. Results The treatment group of 8 cases were coagulated successfully only 1 time by microwave ablation. In the control group, the blood around spleen was increased, and spleen envelope and parenchyma were split to some extent, and blood came out of the rupture of spillover when exploratory laparotomy was done. Conclusions Ultrasound-guided cold circulation microwave ablation is effective in the treatment of classⅡsplenic injury, and it is expected to become a new method of spleen preserving treatment of splenic trauma.
