经阴道三维超声在多囊卵巢综合征超声评价中的价值研究

摘　　要：目的:探讨部分临床指标与超声指标的相关性及经阴道三维超声在多囊卵巢综合征(PCOS)超声评价的价值。方法:研究对象共55例,其中PCOS组35例,对照组20例,记录所有研究对象的体重指数(BMI)并测定相关的内分泌指标,在经阴道三维超声条件下获得卵巢间质的血管化指数(VI)、血流指数(FI)、血管化-血流指数(VFI)及卵巢体积、卵泡数目。结果:PCOS组与对照组比较,BMI、LH、T及VI、FI、VFI、卵巢体积、卵泡数目差异均有统计学意义(P<0.05)。PCOS患者卵巢体积与LH及T值呈正相关(r=0.645,P<0.05;r=0.691,P<0.05),卵泡数目与LH值及卵巢体积呈正相关(r=0.521,P<0.05;r=0.554,P<0.05)。结论:经阴道三维超声测定PCOS患者的卵巢体积与LH、T值及卵泡数目具有相关性。通过测定VI、FI、VFI及卵巢体积、卵泡数目并结合激素水平检测,在超声辅助诊断PCOS上有一定作用。Objective ：To evaluate the value of transvaginal three-dimensional ultrasound in evaluation of Polycystic ovary syndrome （ PCOS）, and the relationship between some clinical parameters and the ultra- sound parameters. Methods：35 PCOS patients and 20 women with normal menstruation were examined by transvaginal three-dimensional and color Doppler ultrasound. The ovarian volume,follicle number, and ovarian stromal vascular parameters including vascular index （ VI ）, flow index （ FI ）, and vascularizational flow index （VFI） ,were recorded. The BMI index and the endocrinal parameters were also examined. Results： BMI, LH, T,VI,FI VFI,ovarian volume,and number of follicles in PCOS group were all significantly different from the control groups. The ovarian volume of PCOS patients was positive related to the luteinizing hormone （LH） （ r = 0. 645, P 〈 0.05）, and ratio of testosterone（T） （ r = 0. 691, P 〈 0. 05）. The follieular number of PCOS pa- tients was positive related to LH and the ovarian volume （r = 0. 525, P 〈 0.05;r = 0. 554, P 〈 0.05 ）. Conclu- sions：The ovarian volume in patients with PCOS determined by transvaginal three-dimensional ultrasound was associated with LH ,T value and number of follicles. Examination of VI, FI ,VFI ,ovarian volume and follicle number by transvaginal three-dimensional ultrasound,in combination of hormone level detection,should con- tribute to the research and diagnosis of PCOS.
