摘 要:目的对比低频脉冲电磁场(PEMF)与阿仑膦酸钠(AL)治疗绝经后骨质疏松症(PMO)的疗效。方法纳入2009年9月-2010年9月初诊为PMO患者40例,利用计算机随机分配为试验组和对照组,所有患者服用相同的基本药物:碳酸钙D3片600 mg/d(钙含量),阿法骨化醇0.5μg/d。试验组加用PEMF,40 min/次,6次/周,5周共30次,结束治疗后随访至12周。对照组加用AL 70 mg/周,持续治疗至12周。分别在治疗前,治疗1、5、12周后,进行骨密度(BMD)、视觉模拟疼痛评分(VAS)、徒手肌力(MMT)评定。结果试验组经过5周的治疗,BMD的增加、疼痛的缓解、肌力的改善与治疗前相比有上升趋势(P<0.05),但与对照组比较差异无统计学意义(P>0.05),组间比较发现试验组腰背肌肌力变化程度较对照组差异有统计学意义(P<0.05)。结论与AL治疗比较,短期内PEMF在缓解疼痛,提高骨质量和增加肌力方面具有与AL相同的效应,尤其是提高腰背肌肌力优于AL。Objective To compare the clinical effects of pulsed electromagnetic fields (PEMFs) with oral alendronate in the treatment of postmenopausal osteoporosis. Methods Fourty patients diagnosed to have postmenopausal osteoporosis (OP) from September 2009 to September 2010 were included in our study. They were randomly divided into the experimental group and the control group. All patients were administered the same basic drugs: Caltrate 600 mg and Alfacalcidol 0.5 μg per day. For the experimental group, PEMFs were offered 6 times per week for 5 weeks (30 times in total), and patients in this group were followed up for 12 weeks. For the control group, alendronate was given at a dose of 70 mg per week for 12 weeks. Bone mineral density (BMD), visual analogue scale, and manual muscle testing (MMT) scale were evaluated before, 1 week, 5 weeks, and 12 weeks after intervention. Results Increasing of BMD, pain relieving, and improvement of MMT had a trend of increasing values after 5 weeks of treatment in the experimental group (P 〈 0.05), but there was no significant difference between the two groups (P 〉 0.05). However, there was a significant difference between the two groups in patients' lower back muscle strength after treatment (P 〈 0.05). Conclusion PEMFs have the same effect as alendronate in pain relief, and bone mass and muscle strength improvement, and are even advantageous in increasing back muscle strength compared with alendronate.
